بررسی علت تفاوت شدت بیماری کووید-19 و مرگ ناشی از آن در میان زنان و مردان: یک مقاله مروری

سابقه و هدف: بیماری کووید-19 در اثر آلوده‌شدن با ویروس sars-cov-2 ایجاد می‌گردد. شیوع بیماری بین جمعیت مردان و زنان تقریباً یکسان است؛ ولی میزان مرگ‌ومیر در مردان بیشتر از زنان گزارش شده است. این مطالعه مروری به دیدگاه‌های سلولی و مولکولی دخیل در تفاوت میزان شدت و مرگ بیماری کووید-19 بین زن و مرد پرداخته است.مواد و روش‌ها: مقالات منتشرشده در پایگاه‌های اطلاعاتی science direct، google،google scholar  وpubmed  طی سال 2019 تا 2021 براساس کلیدواژه‌ها به‌صورت تکی و یا ترکیبی شامل «covid-19»، «sars-cov-2l»، «sex differences»، «gender»، «gender differences»،  «severity» و «mortality» جمع‌آوری شدند و پس از حذف مقالات مشابه و غیرمرتبط، مابقی مورد بررسی قرار گرفت. نتایج: اکثریت مطالعات بیان کرده‌اند که میزان شیوع مبتلاشدن به بیماری کووید-19 بین مردان و زنان تقریباً برابر است؛ در حالی‌که شدت بیماری و مرگ‌ومیر حاصل از این بیماری در مردان به‌طور قابل‌توجهی بیشتر از زنان می‌باشد. نتیجه‌گیری: مجموعه‌ای از مسائل ژنتیکی، ایمونولوژیکی، تفاوت در ترشح هورمون‌ها و سبک زندگی می‌توانند به‌عنوان عواملی برای اختلاف میزان شدت بیماری، و مرگ‌و‌میر در دو جنس در نظر گرفته شوند. قوی‌تربودن سیستم‌ایمنی ذاتی و اختصاصی در زنان، فرار از غیرفعال‌سازی کروموزوم x در نواحی ژن‌های مربوط به سیستم‌ایمنی و اثر تقویت‌کنندگی استروژون بر سیستم‌ایمنی زنان می‌توانند از جمله عوامل مهم در میزان کم مرگ‌و‌میر در زنان مبتلا به کووید-19 در مقایسه با مردان باشند. به‌علاوه عوامل محیطی همچون مصرف سیگار نیز می‌تواند در شدت و مرگ‌ومیر حاصل از بیماری کووید-19 که در مردان مصرف بیشتری دارد، تاثیرگذار باشد.

evaluation of difference of severity and mortality of covid-19 disease in terms of gender: a review paper

background: covid-19 disease is caused by infection with the sars-cov-2 virus. the prevalence of the disease is almost similar between men and women, but the mortality rate is higher in men than women. this study assesses the cellular and molecular aspects involved in the difference of severity and mortality of covid-19 disease between men and women.materials and methods: papers published in science direct, google, google scholar and pubmed databases during 2019 to 2021 based on keywords individually or in combination including covid-19, sars-cov-2, sex differences, gender, gender differences, severity and mortality were collected and after deleting similar papers and irrelevant papers, the rest were assessed.results: the majority of studies have shown that the prevalence of covid-19 disease is approximately equal between men and women, while the severity of the disease and the resulting mortality in men are significantly higher than in women.conclusion: a combination of genetic and immunological factors, differences in hormone secretion, and lifestyle can be considered as factors which differences in disease severity, and mortality between the sexes. stronger innate and specific immune systems in women, escape from x-chromosome inactivation in areas of immune-related genes, and the estrogen-boosting effect on the female immune system can be important factors in low mortality of covid-19 disease in women compared to men. in addition, environmental factors such as smoking can affect the severity and mortality of covid-19 disease, which is more common in men.
