بررسی تاثیر درمان گروهی مبتنی بر پذیرش و تعهد بر خودکارآمدی و هموگلوبین گلیکوزیلهی بیماران مبتلا به دیابت نوع دو

چکیده
|
سابقه و هدف: دیابت، یک بیماری مزمن و متضمن خطر بالای ناتوانی و مرگ است. بهخصوص هنگامیکه سبک زندگی فرد مناسب نبوده، پیروی از درمان پزشکی کم یا نامنظم شود و او پایبند به انجام رفتارهای خودمراقبتی نباشد. بنابراین هدف این مطالعه، بررسی تاثیر درمان act بر خودکارآمدی و hba1c بیماران مبتلا به دیابت نوع دو بود.مواد و روشها: این مطالعه بهصورت کارآزمایی بالینی میباشد. با روش نمونهگیری در دسترس، تعداد 60 نفر از مراجعهکنندگان به بیمارستان صنعت نفت انتخاب شدند و بهصورت تصادفی در دو گروه مداخله و کنترل قرار گرفتند. گروه مداخله علاوهبر درمان پزشکی روتین خود، درمان actرا هم در 8 جلسه درمانی 120 دقیقهای بهصورت هفتهای یک جلسه دریافت کردند. در حالیکه گروه کنترل فقط درمان پزشکی روتین خود را ادامه دادند. زمان اجرای پرسشنامهی خودکارآمدی و انجام آزمایش خون در هر دو گروه، قبل و بعد از اجرای پژوهش بود.نتایج: جهت تجزیه و تحلیل دادهها، از آزمون تحلیل کوواریانس استفاده شد. نتایج نشان داد در گروه مداخله نسبت به گروه کنترل سطح خودکارآمدی بهطور معناداری بالاتر (0/718 ±44/66، p<0.001) وhba1c بهطور معناداری پایینتر (0/032 ±1/33، p<0.001) میباشد. نتیجهگیری: با توجه به یافتههای حاصل، درمان act بر افزایش خودکارآمدی و کاهش hba1cدر مبتلایان به دیابت نوع دو و درنهایت کاهش عوارض و مرگومیر ناشی از این بیماری، موثر است.

Abstract
|
Background: Diabetes is a chronic disease which involves a high risk of disability and death, especially when the lifestyle of the patient doesn rsquo;t fit the patient rsquo;s illness, following up to medical treatment is low or irregular and the person does not adhere to selfcare behaviors. Therefore, this study aimed to investigate the effectiveness of group acceptance and commitment therapy (ACT) on selfcare and HbA1C in type II diabetes.Materials and Methods: This experimental research used an available sampling with pretest posttest design with control group. 60 clients of diabetes who were referred to the oil industry hospital were randomly assigned to intervention and control groups. The intervention group, in addition to its routine medical treatment, received ACT treatment in 8 sessions of 120minute in weekly sessions. While the control group only continued their routine medical treatment. The execution time of selfcare questionnaire was weekly and blood test in both groups was before and after the implementation of the study.Findings: Covariance analysis tests was used to analyze data of HbA1C and repeated measurements was used for selfcare dates. Results showed that in the intervention group, the level of HbA1C was significantly lower (P<0.001) and Selfcare behaviors increased significantly every 8 weeks (P<0.001) compared to the control group.Conclusion: Considering the findings of this study, the acceptance and commitment therapy (ACT) can be effective in improving the type2 diabetes selfcare and reducing levels of HbA1C leading to decreased morbidity and mortality rate.
