Introduction: hepatocerebral degeneration caused by chronic liver insufficiency is a rare condition with the pathophysiology of a possible manganese toxification. its clinical presentation includes parkinsonism, walking ataxia and psychiatric findings. case report: a 78-year-old female patient was admitted with 5-year-long sustaining complaints of imbalance with increasing severity, falls and low-speed walking. the patient had suffered from hbv-related chronic liver failure. t1 sequences of cranial mri inspection revealed hyperintensity changes in basal ganglion and mesencephalic substantia nigra regions. the blood manganese level of the patient was measured and was found to be 433 nmol/l (normal range: 73-210 nmol/l). the patient received iv cana2edta. upon evalution at the third month of treatment bradykinesia was apparently gone, the walking ataxia and rigidity were milder, but the restricted upward sight sustained. conclusion: numerous researches on the treatment have been published. our case with manganese-related chronic acquired dramatically hepatocerebral degeneration responded well to the cana2edta treatment.
