چکیده    Background: fat embolism syndrome (fes) is associated with cerebral dysfunction,respiratory distress and petechia. we report a case of cerebral fat embolism (cfe) secondary to mine explosion with an unexpected out come and typical mri view. summary of case: a 58 year old man was referred to our hospital after a mine explosion. his right leg was successfully amputated below the knee under spinal anesthesia. within the first hours,gcs decreased and the posture progressed to decerebrate. mri t2 weighted images,24 hours after the operation,showed numerous scattered hyperintens foci. then,petechial rashes appeared. despite providing supportive cares used by previous authors the patient deteriorated to vegetative state and expired after 2 months. conclusion: a combination of trauma,respiratory distress,petechial rash and impaired consciousness after a lucid interval should bring up cfe diagnosis. brain ct scan is nonspecific but mri (dwi,flair and t2 sequences) with typical starfield pattern is highly suggestive of this syndrome. patients with massive cfe may not respond to current supportive managements.
