تعیین گونههای مولد لیشمانیوز جلدی در استان کهگیلویه و بویراحمد با روش مولکولی: یک مطالعه گذشته نگر

چکیده
|
زمینه و هدف: لیشمانیوز جلدی یا سالک، در ایران به وسیله دو گونه از انگل لیشمانیا شامل l.major و l.tropica ایجاد میشود. در طی سالهای اخیر، این بیماری در مناطق مختلف استان کهگیلویه و بویراحمد گزارش شده است. با توجه به تفاوتهای فاحش در نوع مخازن حیوانی و ناقل بیولوژیک، شناسایی عامل ایجاد کننده بیماری میتواند در برنامهریزیهای لازم جهت کنترل بیماری موثر باشد. لذا هدف از این مطالعه تعیین گونههای مولد لیشمانیوز جلدی در استان کهگیلویه و بویراحمد با روش مولکولی: یک مطالعه گذشته نگر بود. روش بررسی: در این مطالعه توصیفی مقطعی و گذشته نگر برای تعیین گونه لیشمانیا، تعداد 52 نمونه لام رنگآمیزی شده با رنگ گیمسا از آزمایشگاههای مختلف در سطح شهرهای استان، بین سالهای 1393 تا 1397 جمعآوری و با استفاده از میکروسکوپ نوری از نظر وجود جسم لیشمن بررسی شدند. پس از استخراج dna موجود در سطح لامها، با استفاده از پرایمرهای اختصاصی linr4 و lin17 با روش pcr گونه انگل لیشمانیا شناسایی گردید. دادهها با استفاده از آزمون آماری کای اسکور تجزیه و تحلیل شدند.یافتهها: از کل 52 نمونه با زخمهای لیشمانیوز جلدی، 26 نمونه(50 درصد) مربوط به افراد مذکر و 26 نمونه(50درصد) مربوط به افراد مونث بودند. میانگین سن افراد در مطالعه 23/32 سال( با انحرافمعیار18/42 ±) بود. از کل 52 نمونه، آماستیگوت انگل(جسم leishman) در 36 مورد(69/2 درصد) آنها با مشاهده مستقیم به کمک میکروسکوپ نوری رویت شد و 43 نمونه(82/7 درصد) در pcr مثبت شد. گونه آلوده کننده در تمامی افراد ساکن دهدشت و یاسوج l.major و در گچساران l.tropica بود. نتیجهگیری: نتایج به دست آمده از این مطالعه نشان داد که گونهی l.major عامل لیشمانیوز جلدی در دهدشت و یاسوج و گونهی l.tropica در گچساران میباشند. بنابر این مبارزه با مخازن حیوانی و قطع زنجیره انتقال بر اساس نوع انگل به دست آمده در این تحقیق مورد نیاز است.

Identification of Cutaneous Leishmaniasis-causing Species Using Semi-Nested PCR in Kohgiluyeh and Boyer-Ahmad Province During 2013-2019

Abstract
|
Background aim: Cutaneous leishmaniasis or leishmaniasis in Iran is caused by two species of Leishmania parasites including: L. major and L. tropica. In recent years, the disease has been reported in different parts of Kohgiluyeh and BoyerAhmad Province. Due to the huge differences in the type of animal reservoirs and biological carriers, identifying the causative agent can be effective in the necessary planning to control the disease. Therefore, the aim of the present study was to determine the causative species of cutaneous leishmaniasis in Kohgiluyeh and Boyer Ahmad province, Iran, by molecular methods: a retrospective study. Methods: The present crosssectional and retrospective descriptive study was conducted to determine the species of Leishmania, 52 samples of smears stained with Giemsa dye were collected from different laboratories in the cities of the province between 2013 and 2017 and were examined using light microscope for the presence of Leishman #39;s body. After extracting the DNA present on the surface of slides, using specific primers LINR4 and LIN17, the species of Leishmania parasite was identified by PCR method. The data were analyzed using chiscore statistical test. Results: Out of 52 samples with cutaneous leishmaniasis lesions, 26 (50%) were male and 26 (50%) were female. The mean age of the participants in the study was 23.32 years (with a standard deviation of ±18.42). Out of 52 samples, parasitic amastigotes (Leishman #39;s body) were observed in 36 cases (69.2%) by direct observation using a light microscope and 43 samples (82.7%) were positive for PCR. Infectious species in all residents of Dehdasht and Yasuj were L. major and L. tropica in Gachsaran. Conclusion: the results of the present study indicated that L.major and L.tropica species were the cause of cutaneous leishmaniasis in Dehdasht, Yasuj, and Gachsaran. Therefore, further studies are suggested to identify vectors and reservoirs of cutaneous leishmaniasis parasites in Kohgiluyeh and BoyerAhmad province, Iran.
