مقایسه اثر کرم خرفه و لانولین بر درد نوک پستان زنان شیرده: کارآزمایی بالینی تصادفی شده

مقدمه: درد نوک پستان، یکی از شکایات شایع مادران در روزهای اول بعد از زایمان و دومین علت قطع زودهنگام شیردهی می‌باشد. لانولین، درمان توصیه شده جهت بهبود درد نوک پستان است. از طرفی خرفه به دلیل ترکیبات فلاونوئید و ساپونین، دارای اثرات ضد دردی و ضد التهابی ثابت شده‌ای می باشد. لذا مطالعه حاضر با هدف تعیین و مقایسه اثر کرم خرفه با لانولین بر درد نوک پستان در زنان شیرده انجام شد.روش‌کار: این مطالعه کارآزمایی بالینی تصادفی شده دو سوکور در سال 1395 بر روی 86 زن شیرده مبتلا به درد نوک پستان در شهر مشهد انجام شد. افراد به صورت تصادفی در دو گروه 43 نفری کرم خرفه و لانولین قرار گرفتند. روش صحیح شیردهی و طریقه استعمال دارو به افراد هر دو گروه آموزش داده شد. نمره درد نوک پستان قبل از شروع مداخله و روزهای سوم و هشتم بعد از درمان با استفاده از مقیاس عددی درد توسط پژوهشگر اندازه‌گیری و ثبت شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار آماری spss (نسخه 22) و آزمون‌های آماری تی، فیشر، کای دو و ویلکاکسون انجام شد. میزان p‌ کمتر از 0.05 معنی‌دار در نظر گرفته شد. یافته‌ها: دو گروه قبل از مداخله از نظر شدت درد همگن بودند. اختلاف معناداری در شدت درد در هر یک از گروه‌ها در روز سوم و هشتم نسبت به قبل مشاهده شد و شدت درد در هر دو گروه کاهش یافته بود (p<0.001). شدت درد بین دو گروه در روز سوم و هشتم اختلاف معناداری داشت؛ به‌طوری‌که شدت درد در گروه خرفه کمتر از گروه لانولین بود (p<0.001). نتیجه‌گیری: کرم خرفه در بهبود درد نوک پستان موثرتر از لانولین می‌باشد.

Comparison of purslane cream and lanolin on nipple pain among breastfeeding women: A randomized clinical trial

Introduction: Maternal nipple pain is one of the common complaints on the first few days post birth, which is the second cause of early discontinuation of breastfeeding. Lanolin is the recommended treatment for relieving the nipple pain. In addition, purslane has analgesic and antiinflammatory effects due to containing flavonoids and saponins. Regarding this, the present study was conducted with the aim of determining the effect of purslane cream and lanolin on nipple pain among the breastfeeding women.Material and Methods: This doubleblind, randomized controlled trial was conducted on 86 breastfeeding women suffering from nipple pain in Mashhad, Iran, in 2016. The samples were randomly assigned into two groups of purslane cream and lanolin (n=43 cases in each group). Both groups were trained about the correct method of breastfeeding and using cream. The nipple pain scores were measured and recorded by the researcher before the treatment, as well as 3 and 8 days postintervention using the numeric pain scale. Data analysis was performed in SPSS software (version 22) using the ttest, Fisher's exact test, Chisquare test, and Wilcoxon test. Pvalue less than 0.05 was considered statistically significant.Results: According to the results, the two groups were comparable before the intervention in terms of pain severity. The two groups showed a significant difference in pain severity on days 3 and 8 postintervention, compared to the preintervention stage; accordingly, the severity of pain was decreased in both groups (P>0.001). Furthermore, there was a significant difference between the two groups regarding the pain severity on the third and eighth days postintervention (P>0.001). In this regard, the purslane group showed a lower pain severity than the lanolin group.Conclusion: As the findings of the present study indicated, purslane cream was more effective in the treatment of nipple pain than lanolin.
