دیابت بارداری و اختلال تکامل عصبی در فرزند: یک مرور سیستماتیک

مقدمه: دیابت بارداری، یکی از عوارض جدی دوران بارداری می‌باشد که می‌تواند منجر به پیامدهای نامطلوب مادر و نوزاد در کوتاه‌مدت و طولانی‌مدت شود. مطالعه مرور سیستماتیک حاضر با هدف بررسی ارتباط دیابت بارداری با اختلال تکامل عصبی در فرزند انجام شد.روش‌کار: در این مطالعه مروری جهت یافتن مقالات مرتبط، پایگاه های اطلاعاتی pubmed، scopus، web of science و google scholar با کلیدواژگان مرتبط نظیر دیابت بارداری، کودک، اختلال شناختی و اختلال رفتاری تا مارس 2022 مورد جستجو قرار گرفتند. جهت بررسی کیفیت مطالعات از چک‌لیست اوتاوا استفاده شد. گزارش یافته‌های این مطالعه به‌صورت کیفی ارائه شد.یافته ها: در این مطالعه، 14 مطالعه مشاهده ای با 728140 شرکت‌کننده مورد بررسی قرار گرفتند. جمع‌بندی یافته های موجود در زمینه بروز عوارض تکامل عصبی در فرزندان مادران مبتلا به دیابت بارداری که با معیارهای مختلف تشخیصی دیابت بارداری در آن ها تشخیص داده شده بود، حاکی از احتمال بروز عوارض تکامل عصبی - شناختی در فرزندان مادران مبتلا به دیابت بارداری بود. رایج‌ترین روش تشخیص دیابت بارداری، معیار carpenter - coustan و پس از آن معیار تشخیصی سازمان جهانی بهداشت بود.نتیجه گیری: شواهد در دسترس به‌طور عمده بیانگر ارتباط دیابت بارداری با عوارض عصبی - شناختی در فرزند بودند، اما به‌دلیل محدودیت مطالعات، امکان مقایسه پیامدهای حاصل از روش های تشخیصی و درمانی مختلف امکان‌پذیر نبود.

gestational diabetes and neurodevelopmental disorders in offspring: a systematic review

introduction: gestational diabetes (gdm) is one of the serious complications of pregnancy which can lead to adverse outcomes for mother and offspring in the short and longterm. this systematic review study was performed to investigate the relationship between gdm and neurodevelopmental disorders in the offspring.methods: in this review study, to find the related articles, databases of pubmed, scopus, web of science, and google scholar were searched using the keywords of gestational diabetes, offspring, cognitive disorder and behavioral disorder up to march 2022. the newcastleottawa scale was used to assess the quality of the studies. the findings of this study were reported qualitatively.results: this study reviewed 14 observational studies with 728140 participants. the findings of included studies on the incident of neurodevelopmental complications in the offspring of mothers with gdm, who were diagnosed using different diagnostic criteria, show the possibility of neurodevelopmental complications in the offspring of mothers with gdm. the most common diagnosing criterion of gdm was the carpentercoustan criterion; after that, the world health organization diagnostic criterion was the most common.conclusion: the available evidence mainly indicated the association between gdm and neurodevelopmental disorders in the offspring, while due to limited studies, it was not possible to compare the outcomes of different diagnostic and therapeutic approaches.
