چکیده
|
مقدمه: تغییرات واضح اسید باز نوزاد در دقایق اولیه میتواند بر وضعیت همودینامیک نوزاد اثرات نامطلوبی بر جای بگذارد و سرمهای مختلف تاثیرات متفاوتی بر وضعیت اسید باز میگذارند، لذا مطالعه حاضر با هدف بررسی مقایسه اثرات سرم کلرید سدیم 0.9% با سرم رینگر لاکتات بر وضعیت اسید باز مادر و نوزاد در سزارین الکتیو انجام شد.روشکار: این مطالعه کارآزمایی بالینی تصادفی شده و سهسوکور در سال 98-1397 با مشارکت 48 زن باردار انجام شد. گروه اول سرم سدیم کلراید 0.9% و گروه دوم سرم رینگر لاکتات را دریافت نمودند. نمونه خون شریانی قبل از سزارین (مادر) و پس از سزارین (مادر و نوزاد) و نمره آپگار نوزاد در دقایق اول و پنجم ثبت و بین دو گروه مقایسه شد. تجزیه و تحلیل دادهها با استفاده از نرمافزار آماری spss (نسخه 21) و آزمونهای تی مستقل، تی زوجی، آزمون کای دو، منویتنی، آنالیز واریانس و اندازههای تکراری انجام شد. میزان p کمتر از 0.05 معنیدار در نظر گرفته شد.یافتهها: تمامی فاکتورهای مورد بررسی در abg بین دو گروه مادران معنیدار بود که نشان از بروز اسیدیته بالاتر در گروه کلرید سدیم 0.9% نسبت به گروه رینگر لاکتات داشت (0.05>p)؛ از طرفی بین فاکتورهای بررسی شده در نوزادان (abg و نمرات آپگار) دو گروه تفاوت معنیداری مشاهده نشد (0.05

The effect of Ringer’s lactate serum with 0.9% sodium chloride on maternal and neonatal acid status in elective cesarean section: a triple-blind randomized clinical trial

Abstract
|
Introduction: Clear changes in neonatal acidbase in the early minutes can have adverse effects on the hemodynamic status of the neonate; different serums have different effects on the acidbase status. Therefore, this study was performed with aim to compare the effects of 0.9% sodium chloride serum with Ringer lactate serum on maternal and neonatal acidbase status in elective cesarean section. Methods: This tripleblind randomized clinical trial study was performed with participation of 48 pregnant women in 20182019. The first group received 0.9 sodium chloride serum and the second group received ringer lactate serum. Arterial blood samples before cesarean section (mother) and after cesarean section (mother and baby) and neonatal Apgar score in the first and fifth minutes were recorded and compared between the two groups. Data were analyzed by SPSS software (version 21) and independent ttest, paired ttest, Chisquare test, MannWhitney test, analysis of variance and repeated measures. P<0.05 was considered statistically significant. Results: All factors studied in ABG were significant between the two groups of mothers, indicating a higher acidity in the 0.9% sodium chloride group than the ringerlactate group (p<0.05). On the other hand, there was no significant difference between the studied factors in neonates (ABG and Apgar scores) between the two groups (p>0.05).Conclusion: 0.9% sodium chloride compared to lactated ringer causes acidosis in mothers undergoing elective cesarean section with spinal anesthesia, but none of them has a significant effect on the acidbase status of the infant. Ringer’s lactate serum seems to be a good alternative to 0.9% sodium chloride serum, especially in mothers who are at higher risk for acidosis.
