بررسی میزان مقاومت آنتیبیوتیکی آسینتوباکتر بومانی جدا شده از بیماران بستری شده در بیمارستانهای قائم (عج) و امام رضا (ع) مشهد نسبت به کلیستین و ایمیپنم به روش Etest

چکیده
|
مقدمه: آسینتوباکتر بومانی، یکی از عوامل مهم عفونتهای بیمارستانی است. افزایش مقاومت آنتیبیوتیکی، نگرانیها را درباره درمان این عفونتها افزایش داده است. مطالعه حاضر با هدف تعیین حساسیت آسینتوباکتر بومانی نسبت به کلیستین و ایمیپنم با استفاده از حداقل غلظت مهاری (mic) انجام شد. روشکار: در این مطالعه 2745 نمونه (2745 پلیت) از بیماران بخشهای مختلف بیمارستانهای قائم (عج) و امام رضا (ع) شهر مشهد در فاصله مهر 1395 تا مهر 1396 جمعآوری گردید. در صورتیکه نمونه زخم بود، با استفاده از سواپ استریل پس از ضد عفونی کردن سطح زخم، از قسمت عمقی نمونهبرداری انجام گرفت. در مورد ادرار، وسط ادرار در یک ظرف استریل جمعآوری شد و نمونه برنش توسط پزشک توسط سرنگ نمونه گرفته شد. 100 آسینتوباکتر بومانی از نمونهها با تستهای بیوشیمیایی همچون محیط کلیگلر، اوره، سیترات، تست اندول و لیزین شناسایی گردید. بررسی حساسیت به آنتیبیوتیک با دیسک دیفیوژن انجام گرفت و توسط mic (نوارهای etest) ارزیابی شد. یافتهها: نتایج دیسک دیفیوژن مقاومت کامل نمونههای جدا شده به ایمیپنم را نشان داد که بهوسیله نوارهای etest این نتیجه تایید گردید. تمامی نمونهها به کلیستین حساس بودند. کمترین میزان mic بهدست آمده 25/0 میکروگرم بر میلیلیتر و بیشترین میزان mic، 2 میکروگرم بر میلیلیتر بود که بر اساس استاندارد clsi، این نمونهها حساس در نظر گرفته شدند. نتیجهگیری: مقاومت بالای آسینتوباکتر بومانی به ایمیپنم، نشاندهنده تغییر الگوی مقاومت این باکتری و ناکارآمدی آنتیبیوتیک مذکور برای درمان است، اما کلیستین همچنان موثر میباشد. برای جلوگیری از گسترش گونههای مقاوم به کلیستین، باید از این دارو حدالمقدور کمتر استفاده شود تا مقاومت نسبت به این دارو در آینده ایجاد نگردد.

Study of Imipenem and Colistin resistance rates of isolated Acinetobacter baumannii from hospitalized patients in Imam Reza and Ghaem hospitals of Mashhad through Etest

Abstract
|
Introduction: Acinetobacter baumannii is one of the primary causes of nosocomial infections. Furthermore, the increment of antibiotic resistance has raised concerns about the proper therapeutic approach to this problem. This study aimed to determine the susceptibility of A.baumannii to colistin and imipenem through minimal inhibitory concentration (MIC).Methods: In this study, 2745 samples (2745 plate) were collected from patients in different wards of Ghaem Hospital and Imam Reza Hospital in Mashhad between October 2015 and September 2016. In the case of a specimen was wound, after disinfection of the wound surface using a sterile swab from the inferior part of the sample, middle stream urine was collected in sterile container, and the bronchial specimen was taken by the doctor by syringe. In this study, 100 A.baumannii were isolated from 100 hospitalized patients at Imam Reza and Ghaem hospitals of Mashhad, By Biochemical tests (Kligler iron agar, Lysine, urea indole) diagnostic and then disc diffusion method, and Etest strips were utilized to evaluate infection, antibiotic susceptibility, and MIC, respectively..Results: The obtained results of disc diffusion showed that all isolates were resistant to imipenem, which was also confirmed by Etest strips. Moreover, all isolates were sensitive to colistin. The obtained colistin MIC was 0.25μg/ml for one sample and ≤2μg/ml for others; which were considered sensitive according to the Clinical and Laboratory Standards Institute standards.Conclusion: The high resistance of A.baumannii to imipenem was indicative of resistance pattern changes of this bacteria and insufficiency of the mentioned antibiotic for treatment. However, colistin is still effective and should be prescribed more carefully to prevent resistant strains spread.
