تاثیر برنامه مبتنی بر توانمندسازی پرستاران بر خود مراقبتی و کنترل hba1c در سالمندان مبتلابه دیابت ملیتوس نوع دو مقیم مراکز سالمندی اصفهان در سال 1399

مقدمه: امروزه افزایش شیوع دیابت و عوارض آن در سالمندان به‌عنوان یکی از مشکلات چالش برانگیز بخش سلامت است. از مهم‌ترین استراتژی‌ها مدیریت دیابت در سالمندان آموزش رفتارهای خود مراقبتی می‌باشد که نیازمند پرستاران توانمند در زمینه آموزش دیابت است. مطالعه حاضر با هدف تعیین تاثیر توانمندسازی پرستاران بر خود مراقبتی و کنترل hba1c سالمندان مبتلابه دیابت نوع دو مقیم در مراکز سالمندی انجام شد.روش کار: پژوهش نیمه تجربی حاضر به صورت تک گروهی بر روی 32 نفر از پرستاران و 30 نفر از سالمندان مبتلابه دیابت نوع دو مقیم در سرای سالمندان به شیوه نمونه‌گیری در دسترس انجام شد. پرستاران در کارگاه یک روزه توانمندسازی در زمینه دیابت نوع دو شرکت کردند. ابزار جمع‌آوری داده‌ها پرسشنامه محقق ساخته از محتوای کارگاه جهت سنجش توانمندی پرستاران، پرسشنامه خود مراقبتی توبرت و گلاسکو و شاخص آزمایشگاهی hba1c برای سالمندان بود. داده‌ها با استفاده از spss نسخه 20 در سطح معناداری 0.05 تجزیه‌وتحلیل شدند. پژوهش حاضر با کد اخلاق ir.iau.khuisf.rec.1398.190 در کمیته اخلاق دانشگاه آزاد اسلامی - واحد اصفهان (خوراسگان) به ثبت رسیده است.یافته‌ها: بر اساس یافته‌ها میزان توانمندی پرستاران بعد از کارگاه نسبت به قبل از آن دارای تفاوت آماری معنی‌دار بود (p=0.001). میانگین نمرات سالمندان قبل و سه ماه بعد از توانمندسازی پرستاران از نظر رفتارهای خود مراقبتی (p=0.000) و شاخص hba1c (p=0.000) معنی‌داری بود.نتیجه‌گیری: نتایج نشان داد که توانمندسازی پرستاران دارای پتانسیل‌های مطلوبی جهت تغییر برآیندهای خود مراقبتی در سالمندان مبتلا به دیابت نوع دو مقیم در سرای سالمندان است و با تمرکز بر رویکرد توانمندسازی پرستاران می‌توان گام‌های مثبتی در مدیریت دیابت و hba1c این گروه برداشت. 

the effect of a program based on empowering nurses on self-care and hba1c control in elderly people with diabetes mellitus type 2 living in isfahan nursing homes in 2019

introduction: today, the increasing prevalence of diabetes and its complications in the elderly is one of the most challenging problems in the health sector. one of the most important strategies for managing diabetes in the elderly is teaching self-care behaviors, which requires competent nurses in the field of diabetes education. the present study was conducted with the aim of determining the effect of empowering nurses on self-care and hba1c control of elderly people with type 2 diabetes living in nursing homes.methods: this quasi-experimental study was conducted as a single group on 32 nurses and 30 elderly people with type 2 diabetes residing in nursing homes by convenience sampling. nurses participated in a one-day empowerment workshop on type 2 diabetes. data collection tools were a researcher-made questionnaire of workshop content to measure nurses’ empowerment, tobert and glasgow self-care questionnaire and laboratory index hba1c for the elderly. data were analyzed using spss version 20 at the 0.05 level. the present research has been registered with the ethics code ir.iau.khuisf.rec.1398.190 in the ethics committee of islamic azad university - isfahan branch (khorasgan).results: based on the findings, there was a significant difference between nurses’ empowerment after the workshop compared to before (p=0.001). the mean scores of elderly before and three months after nurses’ empowerment were significant in terms of self-care behaviors (p=0.000) and hba1c index (p=0.000).conclusions: the results showed that the empowerment of nurses has desirable potential to change the outcomes of self-care in elderly people with type 2 diabetes resident in nursing homes, and by focusing on the approach of empowering nurses, positive steps can be taken in the management of diabetes and hba1c of this group.
