بررسی میزان رعایت استاندارد کشوری ترالی احیاء در بیمارستان‌های آموزشی شهر یزد در سال 1403: یک مطالعه مقطعی

زمینه و هدف: مدیریت موثر و ایمن ایست قلبی- ریوی در بیمارستان متکی به دسترسی فوری به داروها، وسایل و تجهیزات اورژانسی است. ترالی احیا، مجموعه‌ای ازکشوها و قفسه‌ها روی چرخ‌ها است که برای حمل و نقل، توزیع دارو و تجهیزات در شرایط اورژانسی در نظر گرفته شده است. بررسی دوره‌ای و تکمیل ترالی احیا برای مطابقت با استاندارد برای اطمینان از آماده بودن ترالی در شرایط اورژانسی بسیار مهم است مطالعه حاضر با هدف تعیین مطابقت ترالی احیا با استاندارد کشوری در بیمارستان‌های دانشگاهی شهر یزد در سال 1403 انجام گرفت.روش‌ها: این مطالعه از نوع مقطعی است. نمونه‌های مورد مطالعه 71 ترالی‌های اورژانس بخش‌های بیمارستان بود که پژوهشگر به صورت تصادفی در شیفت‌های مختلف به تمام بخش‌های بیمارستان‌های آموزشی دانشگاهی شهر یزد مراجعه نمود و با چک لیست تدوین شده بر اساس آخرین دستورالعمل استاندارد ترالی‌های احیا، به ارزیابی کیفیت و کمیت دارو، وسایل مصرفی و تجهیزات ترالی‌ها پرداخته شد.یافته‌ها: بر اساس یافته‌ها از نظر مطابقت با دستورالعمل کشوری، داروها در کشوی اول 11/27 درصد و کشوی دوم 19/72 و در کشوی سوم 14/09، کشوی چهارم 40/84 و کشوی پنجم 37/62 موجود نبود از نظر تعداد داروها در کشوی اول 52/11 درصدم کشوی دوم 66/20 درصد و وسایل و تجهیزات در کشوی سوم 25/59 درصد، کشوی چهارم 20/32 درصد و کشوی پنجم 43/26 درصد کافی نبودند. از نظر تاریخ انقضای داروها در کشوی اول 11/27 درصد و کشوی دوم 19/72 درصد و تاریخ انقضای وسایل و تجهیزات در کشوی سوم 14/56 درصد، کشوی چهارم 23/74 درصد و کشوی پنجم 39/83 درصد معتبر نبودند. همچنین از نظر وجود و آمادگی سیستم اکسیژن مرکزی 12/7 درصد و 29/8 درصد، ساکشن 29/6 درصد و 45/1 درصد، دفیبریلاتور 8/5 درصد و 8/5 درصد، پد پیس اکسترنال 47/9 درصد پاسخ منفی بود. کیفیت ترالی با 24 آیتم بررسی شد که در اکثریت موارد مشکلات ساختاری یا کیفیتی در ترالی‌های احیا یافت نشد.نتیجه‌گیری: در مقایسه ترالی‌های احیا با استاندارد کشوری می‌توان استنباط نمود که ترالی‌های احیا به صورت قابل قبول هستند اما کاملاً منطبق بر اساس آخرین دستورالعمل کشوری نگهداری نمی‌شوند. بنابراین در جهت پیروی از آخرین دستورالعمل ترالی احیا، انجام اقداماتی همچون تدوین و بازنگری خط‌مشی کنترل دوره‌ای ترالی احیا و جایگزینی داروها و وسایل، بازنگری استاندارد ترالی احیا بر اساس بخش درمانی و آموزش آخرین دستورالعمل ترالی احیا به کارکنان وجود دارد.

the level of compliance with national standards in resuscitation trolleys in teaching hospitals in yazd in 2024: a cross-sectional study

background & aim: effective and safe management of cardiopulmonary arrest in hospitals relies on immediate access to emergency drugs, equipment, and supplies. a resuscitation trolley is a set of drawers and shelves on wheels that are intended for transporting and distributing drugs and equipment in emergency situations. periodic inspection and completion of the resuscitation trolley to comply with the standard is very important to ensure that the trolley is ready in emergency situations. the present study was conducted to determine the compliance of the resuscitation trolley with national standards in yazd teaching hospitals in 1403.methods: this study was a cross-sectional research. the study samples included 71 emergency trolleys in hospital wards. the researcher randomly visited all departments of university teaching hospitals in yazd city in different shifts and evaluated the quality and quantity of medicines, consumables and equipment of the trolleys with a checklist compiled based on the latest standard guidelines for resuscitation trolleys.results: according to findings, in terms of compliance with the national guidelines, medicines were not available in the first drawer 11.27% and the second drawer 19.72%, and the equipment in the third drawer 14.09%, the fourth drawer 40.84% and the fifth drawer 37.62%. in terms of the number of medicines in the first drawer 52.11%, the second drawer 66.20% and the equipment in the third drawer 25.59%, the fourth drawer 20.32% and the fifth drawer 43.26% were insufficient. in terms of the expiration date of the drugs in the first drawer 11.27% and the second drawer 19.72% and the expiration date of the equipment and devices in the third drawer 14.56%, the fourth drawer 23.74% and the fifth drawer 39.83% were not valid. also, in terms of the presence and readiness of the oxygen capsule or central oxygen system 12.7% and 29.8%, suction 29.6% and 45.1%, defibrillator 8.5% and 8.5%, external pad 47.9%, the response was negative. the quality of the trolley was examined with 24 items, and in most cases, structural or quality problems were not found in the resuscitation trolleys.conclusion: in comparing the resuscitation trolleys with the national standard, it can be concluded that the resuscitation trolleys are acceptable but are not maintained completely in accordance with the latest national guidelines. therefore, in order to follow the latest resuscitation trolley guidelines, measures such as developing and reviewing the policy for periodic control of resuscitation trolleys and replacing drugs and devices, reviewing the resuscitation trolley standard based on the treatment department, and training staff on the latest resuscitation trolley guidelines are being implemented.
