   آمادگی‌های ضروری مراکز درمانی در بحران‌های شیمیایی، بیولوژیکی، رادیولوژیکی، هسته‌ای: یک مرور نظام‌مند

چکیده    زمینه و هدف: جنگ‌ها همواره تهدیدی جدی برای جوامع بشری بوده‌اند. با پیشرفت تکنولوژی، جنگ‌های پیچیده‌تر از جمله حوادث(cbrne) (شیمیایی، بیولوژیکی، رادیولوژیکی، هسته‌ای و انفجاری) به وجود آمده‌اند که خسارات جبران‌ناپذیری بر سلامت عمومی و محیط زیست وارد می‌کنند. این مطالعه به بررسی آمادگی بیمارستان‌ها و مراکز درمانی برای مواجهه با این حوادث می‌پردازد تا اقدامات بهبوددهنده شناسایی شوند.روش‌ها: این مطالعه به صورت یک مرور سیستماتیک و بر اساس دستورالعمل‌های prisma انجام شد. جستجوی منابع در پایگاه‌های اطلاعاتی pubmed/medline، embase، web of science، scopus، cochrane library، sciencedirect و proquest به ‌منظور شناسایی مقالات مرتبط با آمادگی بیمارستان‌ها در مواجهه با حوادث (cbrne) انجام شد و مقالات منتشرشده از ژانویه 2010 تا دسامبر 2023 مورد بررسی قرار گرفتند. معیارهای ورود شامل مطالعات مرتبط با آمادگی بیمارستان‌ها در پاسخ به حوادث (cbrne) بود، در حالی که مقالاتی که بر آمادگی پیش‌بیمارستانی یا حوادث غیرمرتبط متمرکز بودند، حذف شدند. انتخاب مطالعات بر اساس مراحل پیشنهادی tawfik و همکاران برای مرورهای سیستماتیک حوادث انجام شد. علاوه بر این، از چارچوب pico به ‌منظور تدوین پرسش‌های تحقیقاتی مرتبط با جمعیت هدف، مداخلات و نتایج استفاده شد.یافته‌ها: یافته‌های این مطالعه نشان داد که آمادگی بیمارستان‌ها در مواجهه با حوادث (cbrne) به سه حوزه اصلی تجهیزات و زیرساخت‌ها، آموزش پرسنل، و مدیریت و پروتکل‌ها وابسته است. تامین تجهیزات مناسب، برگزاری آموزش‌های تخصصی و تمرین‌های عملی منظم، و تدوین پروتکل‌های به ‌روز، از عوامل کلیدی برای بهبود واکنش به شرایط بحرانی هستند. این نتایج همسو با مطالعات پیشین، بر اهمیت ارتقای مداوم این حوزه‌ها تاکید دارد.نتیجه‌گیری: یافته‌های این پژوهش نشان می‌دهد که آمادگی بیمارستان‌ها برای مواجهه با حوادث (cbrne) به‌ ویژه در زمینه ارتقای تجهیزات حفاظتی، آموزش کارکنان و تدوین پروتکل‌های مدیریتی، نیازمند بهبود است. آموزش‌های مستمر و برگزاری مانورهای عملیاتی می‌تواند نقش مهمی در ارتقای مهارت‌ها و کاهش تلفات در شرایط بحرانی ایفا کند. با توجه به اهمیت آمادگی نظام سلامت در مواجهه با این حوادث، پیشنهاد می‌شود در مطالعات آتی، آمادگی مراکز پیش‌بیمارستانی نیز به ‌طور جامع بررسی شود.

   essential preparedness measures for healthcare centers in chemical, biological, radiological, and nuclear crises: a systematic review

Abstract    background & aim: wars have always posed a significant threat to human societies. with the advancement of technology, more complex conflicts, including chemical, biological, radiological, nuclear, and explosive (cbrne) incidents have emerged, causing irreparable harm to public health and the environment. this study investigates the preparedness of hospitals and healthcare centers to respond to such incidents, aiming to identify measures for improvement.methods: this study was conducted as a systematic review following prisma guidelines. a comprehensive literature search was performed in databases including pubmed/medline, embase, web of science, scopus, cochrane library, science direct, and proquest to identify articles related to hospital preparedness for cbrne incidents. articles published between january 2010 and december 2023 were reviewed. the inclusion criteria focused on studies addressing hospital preparedness for responding to cbrne incidents, while articles centered on prehospital preparedness or unrelated events were excluded. study selection followed the systematic review steps proposed by tawfik et al. for disaster research. additionally, the pico framework was employed to formulate research questions regarding the target population, interventions, and outcomes. results: the findings of this study revealed that hospital preparedness for cbrne incidents depends on three main domains: equipment and infrastructure, personnel training, and management and protocols. key factors for improving critical response include ensuring the availability of appropriate equipment, providing specialized training and regular drills, and developing updated protocols. these results align with previous studies, underscoring the importance of continuous advancements in these areas.conclusion: the results of this research highlight the need to enhance hospital preparedness for cbrne incidents, particularly in terms of improving protective equipment, staff training, and managerial protocol development. continuous education and operational drills can play a crucial role in improving skills and reducing casualties in crisis situations. considering the importance of health system preparedness in addressing these incidents, future studies are recommended to comprehensively assess prehospital preparedness as well.
