بررسی ادراک پرستاران از محیط عملکرد پرستاری در بخش های مراقبت ویژه; یک مطالعه توصیفی- مقطعی

زمینه و هدف: بخش‌های مراقبت ویژه یک محیط پیچیده و پویا با مداخلات چندگانه به منظور ریکاوری بیماران بحرانی و ناپایدار است. ادراک پرستاران از مولفه‌های محیط عملکردی در این بخش، می‌تواند در ابقاء پرستاران خبره و متخصص تاثیرگذار باشد. هدف این مطالعه بررسی ادراک پرستاران از محیط عملکرد حرفه‌ای پرستاری در بخش‌های مراقبت ویژه بیمارستان‌های وابسته به دانشگاه علوم پزشکی شیراز است.روش‌‌ها: پژوهش حاضر یک مطالعه توصیفی مقطعی و چند مرکزی است که در20 بخش مراقبت ویژه از پنج بیمارستان وابسته به دانشگاه علوم پزشکی شیراز بین سال‌های 1398-1397 انجام شده است. جامعه پژوهش را 350 نفر از پرستاران شاغل در بخش‌های مختلف مراقبت ویژه بزرگسالان تشکیل دادند که به روش نمونه‌گیری تصادفی طبقه‌بندی شده متناسب با تخصیص سهمیه‌ای انتخاب شدند. داده‌ها با استفاده از سیاهه محیط عملکرد حرفه‌ای پرستاری (nppei) که در ایران طراحی و اعتباریابی شده است، جمع‌آوری شد.یافته‌ها: میانگین نمره کل محیط عملکرد حرفه‌ای پرستاری 2.5±38.2 به‌ دست آمد. بعد رهبری کارآمد بیشترین میانگین 3.9±45.5 و بعد شفافیت خط مشی‌ها کمترین میانگین نمره 3.1±31.2 را کسب کردند. نمره محیط عملکرد حرفه‌ای پرستاری با اطلاعات دموگرافیک پرستاران ارتباط معنی‌داری نداشت(p>0.05).نتیجه‌گیری: نتایج پژوهش حاضر نشان داد، ادراک پرستاران از محیط عملکرد حرفه‌ای پرستاری در مراقبت ویژه در حد نسبتاً مطلوب است. لذا با توجه به اهمیت ویژگی‌های محیط کار در جذب و ابقاء پرستاران خبره، مدیران پرستاری و سیاست‌گذاران حوزه سلامت باید در جهت پایش مکرر و ارتقاء بخش‌های ویژه تلاش کنند.

Assessing Nurses’ Perception of Nursing Professional Practice Environment in Intensive Care Units; Descriptive Cross-Sectional Study

Background and Aim: Intensive care unit (ICU) is a complex and dynamic unit with multiple interventions to recover critical and unstable patients. Nurses’ perception of the components of the nursing professional practice environment (NPPE) in these units can be effective in retaining qualified and expert nurses.The aim of the study was to investigate the nurses’ perception of the NPPE in ICU hospitals affiliated with the Shiraz University of Medical SciencesMaterials & Methods:The present study is descriptive crosssectional, multicentered study was conducted in 20 ICUs of five teaching hospitals affiliated with the Shiraz University of Medical Sciences between 2018 to 2019. The research population consisted of 350 nurses working in various adult ICUs who were selected by the stratified random sampling with a proportional allocation. Data were collected using the Nursing Professional Performance Inventory (NPPEI) designed and validated in Iran.Results The mean score for the NPPEI was 38. 2±2. 5. “Effective leadership” and “Policy Transparency “domains obtained the highest (45.5±3.9) and lowest (31.2±3.1) scores. There was not significantly correlated between the demographic data and the NPPEI score (p>0.05).Conclusion The results of the present study showed that nurses’ perception of the NPPE in ICU is relatively favorable. Therefore, considering the importance of workplace attributes in attracting and retaining qualified nurses, nursing managers and health policymakers should attempt for frequent monitoring and promotion of the critical wards.
