بررسی تاثیر اجرای آلگوریتم مدیریت درد بر سطح هوشیاری بیماران با کاهش سطح هوشیاری

زمینه و هدف: برنامه‌ های مدیریت درد، بیماران با کاهش سطح هوشیاری، با این دیدگاه که ممکن است مصرف داروهای مسکن، اقامت طولانی در بخش و همچنین هزینه‌های بستری، دارویی و درمانی را کاهش دهد، می‌تواند مفید باشد. لذا مطالعه حاضر با هدف تعیین تاثیر اجرای الگوریتم مدیریت درد بر سطح هوشیاری بیماران با کاهش سطح هوشیاری انجام شد.روش‌ها: در مطالعه کارآزمایی بالینی یک سوکور حاضر ، 90 بیمار با کاهش سطح هوشیاری و تحت تهویه مکانیکی بستری در بخش مراقبت‌های ویژه بیمارستان خاتم الانبیاء شهر زاهدان به شیوه در دسترس انتخاب و با استفاده از بلوک‌های جایگشتی در دو گروه الگوریتم مدیریت سطح هوشیاری(45) و گروه کنترل(45) تخصیص یافتند. الگوریتم مدیریت درد در گروه مداخله در شیفت صبح بین ساعات 108، شیفت عصر ساعات 1715، شیفت شب ساعات 2422 به مدت 24 ساعت اجرا شد. ابزار مطالعه شامل فرم مشخصات جمعیت شناختی، مقیاس رفتاری درد و مقیاس کمای گلاسکو بود.یافته‌ها: نتایج نشان داد میانگین نمره سطح هوشیاری در سه شیفت مورد بررسی بین دو گروه تفاوت معنی‌داری نداشت. اما میانگین حجم داروی مسکن انفوزیون شده در دو شیفت صبح و شب بین دو گروه معنی‌دار بود(p<0.05).نتیجه‌گیری: استفاده از آلگوریتم مدیریت درد به طور قابل توجهی منجر به کاهش حجم داروی مسکن انفوزیون شده به بیماران شد اما بر سطح هوشیاری آنها تاثیری نداشت.

Effect of pain management algorithm on pain intensity in patients with loss of consciousness

Background & Aim: Pain management program can be helpful in patients with loss of consciousness due to decrease in analgesic drugs, prolonged hospital stay; and also decrease in cost of admission, medicine and medical treatment. The current study was done aimed to determine the effect of pain management algorithm on level of consciousness in patients with loss of consciousness .Materials & Methods: In this randomized clinical trial study, 90 patients with loss of consciousness under mechanical ventilation were selected through convenience method and were randomly allocated into two groups of 45 people as the intervention and control groups. Pain management algorithm was implemented in intervention group for 24 hours in morning shift between 810, evening shift between 1517 and in the night shift between 2224. Data collection tools were the demographic information form and Glasgow Coma Scale (GCS) and Behavior Pain Scale (BPS).Results:. In terms of level of consciousness, there was no significant difference between the two groups after the intervention, but the mean of volume of sedation drugs was significant different between the two groups in morning and night shift (p<0.05)Conclusion: The use of the pain management algorithm can significantly impact on volume of sedation but has no effect on level of consciousness.
