چالش‌های اخلاقی مراقبت از کودکان و نوجوانان از دیدگاه متخصصین اطفال

زمینه و هدف: امروزه پزشکان کودکان در بالین با چالش‌ها و دوراهه‌های اخلاقی زیادی رو به رو می‌شوند. پژوهش حاضر با هدف تبیین این چالش‌ها در حوزه مراقبت از کودکان و نوجوانان از دیدگاه متخصصین اطفال انجام شده است.روش: این مطالعه با رویکرد کیفی و روش تحلیل محتوا در سال 1401 انجام شده است. نمونه‌گیری به صورت هدفمند انجام شد و 20 نفر از متخصصین اطفال بیمارستان مرکز طبی کودکان دانشگاه علوم پزشکی تهران در این مطالعه شرکت داشتند. داده‌ها از طریق مصاحبه فردی عمیق و نیمه‌‌ساختارمند جمع‌آوری و با رویکرد تحلیل محتوای مرسوم، تجزیه و تحلیل شدند.ملاحظات اخلاقی: در این مطالعه از شرکت‌کنندگان برای انجام مصاحبه‌ها رضایت اخذ شده است و اسامی هیچ یک از آنان منتشر نشده و برای انجام مطالعه کد اخلاق (ir.tums.chmc.rec.1399.165) اخذ شده است.یافته‌ها: با تجزیه و تحلیل مصاحبه‌ها سه گروه‌ چالش‌های «مشترک بین کودکان و نوجوانان»، «خاص نوجوانان» و «خاص بخش مراقبت ویژه کودکان» انتزاع گردید. چالش‌های مشترک بین کودکان و نوجوانان، شامل سه طبقه «عوامل مربوط به کادر درمان»، «عوامل مربوط به والدین» و «مشکلات سیستمی و سیاستگذاری» می‌باشد. چالش‌های خاص نوجوانان، شامل دو طبقه مشکلات آنان در «جامعه» و «محیط درمانی» می‌باشد. چالش‌های خاص بخش مراقبت ویژه، شامل سه طبقه «تاثیر روحی و روانی بستری در بخش‌های ویژه بر روی کودک و نوجوان و والدین»، «کمبود منابع و امکانات ساختاری در بخش‌های مراقبت ویژه کودکان» و «مشکلات کودکان در شرف مرگ و تازه فوت‌شده» می‌باشد.نتیجه‌گیری: نتایج این مطالعه نشان داد که بسیاری از چالش‌های اخلاقی در طبابت کودکان ناشی از عدم آشنایی کادر درمان و یا والدین با مسائل اخلاقی و حقوقی در مراقبت از کودکان و یا ناشی از مشکلات ساختاری نظام سلامت، از جمله فقدان خط مشی و رویه‌های اجرایی مشخص و یا کمبود منابع و تجهیزات می‌باشد، لذا توجه به آموزش این مسائل و تبیین این خط مشی‌ها و رویه‌ها در راستای اجرای قانون حمایت از اطفال و نوجوانان و تامین منابع لازم عمده‌ترین نیازها و چالش‌هایی است که بایستی به آن پرداخته شود.

ethical challenges of caring for children and adolescents from theperspective of pediatricians

background and aim: today, pediatricians face many ethical challenges and dilemmas in clinical practice. thepresent study has been conducted with the aim of explaining these challenges in the domain of pediatric andadolescent care from the perspective of pediatricians.methods: this study has been conducted through qualitative approach and content analysis method in 2022. thesamples were selected using purposive sampling method and included 20 pediatricians working at children'smedical center hospital, affiliated to tehran university of medical sciences. the data were collected through indepthand semi-structured individual interviews and analyzed using the conventional content analysis approach.ethical considerations: in this study, consent was obtained from the participants before conducting the interviewsand none of their names have been published. besides, the study has been conducted under the code of ethics(ir.tums.chmc.rec.1399.165).results: through analyzing the interviews, three categories of challenges were extracted: the challenges faced byboth children and adolescents, the challenges exclusively faced by adolescents and the challenges existing inthe pediatric intensive care unit. the challenges faced by both children and adolescents include three sub-categories:the factors related to the healthcare staff, the factors related to parents and organizational and policy makingissues. the challenges exclusively faced by adolescents include two sub-categories: the society-related problemsand the problems related to the healthcare environment. the challenges existing in the pediatric intensive care unitinclude three sub-categories: the mental and psychological effects of hospitalization in intensive care units onchildren, adolescents and parents, the lack of resources and structural facilities and the problems related todying and recently lost children.conclusion: the results of this study indicate that many of the ethical challenges in pediatric medicine either stemfrom the healthcare staff or parents’ unfamiliarity with ethical and legal issues, or are the result of structuralproblems in the healthcare system such as the lack of executive policies and procedures or the shortage of resourcesand equipment. therefore, paying attention to teaching these issues and explaining these policies and procedures, inline with the implementation of the law on the protection of children and adolescents and providing the necessaryresources, is the main need and challenge that must be addressed.
