مروری بر تاریخچه‌ی شکل‌گیری رشته‌ی تخصصی پزشکی اجتماعی و ضرورت وجودی آن در نظام سلامت

این مطالعه، مروری روایتی و پژوهشی اسنادی است که با بررسی متون، مستندات، جست‌وجوی مدارک مرتبط چاپی و اینترنتی به دو زبان فارسی و انگلیسی، یافته‌ها را استخراج، دسته‌بندی، تلخیص و گزارش کرده است. هدف از این مطالعه، تعیین روند شکل‌گیری رشته‌ی پزشکی اجتماعی در ایران و جهان و بررسی ضرورت وجودی آن بود. رشته‌ی پزشکی اجتماعی در حدود یک سده پیش، برای پاسخ‌گویی به چالش‌های عمده‌ی زمان خود، مانند نبود دید جامعه‌محور به سلامت و بیماری، وجود شکاف بین پزشکی بالینی و سلامت عمومی و بی‌توجهی به تاثیر تعیین‌کننده‌های اجتماعی سلامت به وجود آمد. متخصصان پزشکی اجتماعی قادرند که بسیاری از مشکلات فعلی نظام سلامت، مانند بیماری‌محوری، نابرابری، تامین مالی ناعادلانه، افزایش روزافزون تقاضا و هزینه‌های خدمات سلامتی را کاهش دهند و پرچم‌دار توجه به تعیین‌کننده‌های اجتماعی و ایجاد ارتباط سازنده بین بخش‌های متولی سلامت با سایر سازمان‌های اثرگذار بر سلامت باشند. این رشته می‌تواند راهگشای نظام سلامت برای فایق‌آمدن بر مهم‌ترین چالش‌های معاصر باشد. بی‌توجهی به این رشته، نظام سلامت را در رسیدن به اهداف توسعه‌ی پایدار و ارتقای سلامت جامعه با مشکل جدی روبه‌رو خواهد ساخت. حمایت بیشتر مسئولان و بهره‌گیری از مهارت و توانمندی‌های متخصصان این رشته می‌تواند موجب پاسخ‌گویی بهتر به نیازهای سلامت جامعه و دستیابی آسان‌تر به اهداف کلان نظام سلامت شود.

Reviewing the History of Community Medicine Specialty Formation and the Necessities of Its Existence in Health System

The purpose of this study was to perform a narrative review and documentary research in the history of community medicine specialty formation and the needs for its existence in health systems. We searched and reviewed related literature and documents in English and or Farsi (printed or online). Data was extracted and findings were categorized, summarized, and reported. About a century ago community medicine specialty was formed to respond to major health challenges of that time such as neglecting the effects of social determinants on populations rsquo; health and total separation between clinical care and public health. Community medicine professionals are able to address many of the current health problems such as inequalities in health, fair financing problems, increasing demands and costs and diseasebased health system approaches. They can be the leading advocates to emphasize the importance of social determinants in health of populations. They can promote communications with influencing institutions outside the health system to improve the health of the communities. This discipline can pave the way for the health systems to tackle the most important contemporary health challenges. Lack of attention to this discipline may hinder the achievement of sustainable development goals and health promotion of the communities. More support from officials and utilizing the skills of community medicine specialists can help health systems not only to better respond to the health needs of the society but also to facilitate the achievement of their main goals.
