ضرورت ملاحظه‌ی حقوق پزشک در گفتمان اخلاق پزشکی

در سال‌های اخیر، در ادبیات اخلاق پزشکی و در خصوص رابطه‌ی پزشک و بیمار، به‌طوری گسترده، به حقوق بیمار پرداخته شده است؛ اما حقوق طرف دیگر این رابطه، کمتر در کانون توجه قرار گرفته و بیشتر، تکالیف او مطرح بوده است. از حقوق پزشک، دو تلقی می‌توان در نظر گرفت؛ حقوقی که در تقابل با حقوق بیماران قرار می‌گیرد و حقِ تعالی و بهبود نفس. در این نوشتار که به‌صورت مقاله‌ای نظری ارائه شده، سعی بر این بوده است تا دلایل این کم‌توجهی و عوارض ناشی از آن، بیان شود. عقیده‌ی سنتی بر وجود قدرت نامتوازن در رابطه‌ی پزشک و بیمار، تاکیدنکردن اصول اخلاق پزشکی به خود پزشک، نبود ساختارهای حمایتی، رواج مشتری‌مداری در پزشکی و رویکرد حرفه مندی افراطی را می‌توان از دلایل بی‌توجهی به حقوق پزشکان در ادبیات اخلاق پزشکی دانست. ادامه‌ی این امر، عوارضی گوناگون در پی دارد؛ عوارضی مانند برهم‌خوردن توازن در کار و زندگی پزشک، شکل‌گیری خشونت نمادین در جامعه‌ی آن‌ها، فرسودگی شغلی و نادیده‌انگاشتن اصل سپاس و قدردانی و درنهایت، رعایت‌نشدن حقوق بیماران. نتیجه اینکه در کنار لحاظ کردن اولویت حقوق بیماران، توجه به حقوق پزشک، به‌نحوی مقوم رعایت حقوق بیمار است که از اهداف اولیه‌ی اخلاق پزشکی به شمار می‌رود و بدین سبب باید مد نظر قرار گیرد.

Need for Considering Rights of Physicians in Medical Ethics Discourse

Over the past years in the literature of medical ethics about the doctorpatient relationship, patient's rights have been greatly considered, but the other side of this relationship is not concerned, and the physician rsquo;s obligations have been highlighted. The two meanings are understood from the concept of physicians rsquo; rights; their rights in conflict with the patient rights rsquo; and the right to self improvement. We want to review the causes of this neglect and its complications. The reasons for not paying attention to the physician rsquo;s rights include: traditional beliefs about the power imbalance in doctorpatient relationship, lack of emphasis in the four principles of medical ethics, lack of supportive structures, customerorientation in medicine, and hyper medical professionalism. Continuing this neglect creates complications that include: physicians rsquo; worklife imbalance, symbolic violence in their society, burnout of physicians, forgetting the principle of gratitude and ultimately, the lack of respect for patients' rights. Consequently, the discourse of medical ethics fails to achieve its primary goals.
