گزارش یک مورد palisaded encapsulated neuroma (pen)در لثه لینگوال مندیبل

مقدمه:palisaded encapsulated neuroma (pen) یک تومور خوش خیم ناشایع با منشا عصبی بوده که شایع ترین محل آن، پوست صورت است و گاهی اوقات ممکن است در حفره دهان، به ویژه زبان دیده شود. بعضی معتقدند این ضایعه، هامارتوماتوز بوده و ممکن است ترومای مکرر، در اتیولوژی آن نقش داشته باشد. گزارش مورد: بیمار یک خانم 27 ساله، دارای تورم بدون درد با سطح صاف در لثه لینگوال سمت راست مندیبل می باشد که ضایعه از یک سال قبل ایجاد گردیده و فاقد نمای رادیوگرافیک است. در تشخیص افتراقی بالینی ضایعه، لیپوما و فیبروما مطرح شدند. بر اساس بررسی هیستوپاتولوژیکی، تومور خوش خیم سلول دوکی گزارش شد، سپس جهت تشخیص قطعی، رنگ آمیزی نقره و نیز رنگ آمیزی ایمونوهیستوشیمیایی برای نشانگرهای desmin، nse، sma و s100  انجام گرفت. به دلیل رنگ پذیری مثبت با s100 و nse و نیز رنگ پذیری مثبت با نقره, تشخیص نهایی pen گذاشته شد. نتیجه گیری: تشخیص دقیق pen و افتراق آن از سایر تومورهای سلول دوکی، می تواند در درمان صحیح آن، کمک کننده بوده و از درمان های غیرضروری جلوگیری کند.

Palisaded Encapsulated Neuroma of the Lingual Mandibular Gingiva: A Case Report

Introduction: Palisaded encapsulated neuroma (PEN) is a rare benign neural tumor, often localized on faces and sometimes observed in a striking predilection on the skin of the face. This tumor might also occur in oral cavity, especially on the tongue. Some researchers have speculated that PEN is hamartomatosis, and there is a possible role of frequent trauma in its etiology. Case Report: Our case was a 27yearold female with a painless flat swell in the right lingual mandibular gingiva. This lesion was formed about a year ago without any radiographic appearance. Fibroma and lipoma were included in the differential diagnosis of the lesion. Benign spindle cell tumor was reported in this reported according to histopathological examination results. Afterwards, immunohistochemical and silver stains were performed on desmin, S100, alphasmooth muscle actin (αSMA) and neuronspecific enolase markers for definitive diagnosis. The final diagnosis was PEN due to positive staining for S100 and NSE, as well as positive silver staining. Conclusion: According to the results, accurate diagnosis of PEN and differentiating it from other spindle cell tumors could help in the selection of the best treatment and prevention of unnecessary procedures.
