بررسی نتایج درمان در دندان های درمان ریشه شده در بخش دانشجویی دانشکده دندانپزشکی مشهد در طی سال های 95-1391

مقدمه: آموزش موفق اندودانتیکس، تثبیت مهارت و دانش دانشجویان یک پایه صلاحیت توانایی انجام درمان ریشه غیرجراحی رضایت بخش می باشد. بنابراین در این مطالعه، هدف اصلی مشخص کردن نتایج درمان و بررسی میزان موفقیت درمان ریشه توسط دانشجویان عمومی دانشکده دندانپزشکی مشهد در طی سال های 95-1391 بود تا اقداماتی جهت بهبود نتیجه درمان صورت بگیرد. مواد و روش ها: پرونده 70 بیماری که توسط دانشجویان دوره عمومی در بخش درمان ریشه دانشکده دندانپزشکی مشهد درمان شده بودند، مورد بررسی قرار گرفت و بیماران جهت پیگیری              فراخوانده شدند. موفقیت درمان ریشه بر اساس معیارهای بالینی و رادیوگرافیک (موفقیت، شکست و غیرقابل ارزیابی) ارزیابی شدند. عوامل موثر دیگری شامل سن، جنس، دندان درمان شده، فک، قابلیت بازسازی تاج، علل نامناسب بودن درمان و واحد درسی نیز بررسی شدند. از آزمون های دقیق فیشر و آنالیز واریانس یک عاملی در تحلیل داده ها استفاده شد.یافته ها: موارد موفقیت آمیز درمان ها 74.3 درصد، موارد شکست 20 درصد و موارد غیرقابل ارزیابی 5.7 درصد گزارش شد و 17.1 درصد دندان ها کشیده شده بودند. دندان هایی که ترمیم مناسب داشتند، موفقیت بیشتری در مقایسه با دندان های ترمیم نشده نداشتند (0.076=p). فاکتورهای سن، جنس، دندان درمان شده و فک هم تاثیر قابل توجهی بر نتیجه درمان نداشتند (0.05p). بیشترین میزان موفقیت در درمان ریشه مناسب و ترمیم تاج سالم دیده شد (96.4 درصد) ولی ارتباط آماری معنی داری وجود نداشت (0.448=p). درواقع نوع ترمیم دندان با نتیجه درمان از نظر آماری ارتباط معناداری نداشت (0.076=p)، ولی از نظر کلینیکی دارای اهمیت بود.نتیجه گیری: میزان موفقیت 70 دندان درمان ریشه شده توسط دانشجویان عمومی دندانپزشکی مشهد در فالوآپ سه ساله 74.3 درصد گزارش شد. ترمیم مناسب دندان پس از درمان ریشه فاکتوری تاثیرگذار بر نتایج درمان می باشد.

Evaluation of the Outcome of Endodontic Treatments Performed by Undergraduate Students of Mashhad Dental School

Introduction: Successful endodontics training is the integration of students’ skills and knowledge as the basis for mastery and competence in the work. Therefore, they can perform satisfactory nonsurgical endodontic treatment on singlerooted and multirooted teeth. The present study aimed to determine the results and evaluate the success rate of endodontic treatment to improve the outcome of treatment conducted by general students of Mashhad Dental School in 20112012.Materials & Methods: A total of 70 records of treated patients were reviewed by students, and they were recalled for followup. The success of endodontic treatment was firstly assessed based on clinical criteria and then based on radiographic criteria in one of three groups: success, failure, and unknown. Other factors including age, gender, tooth area, jaw, postrestoration, causes of inappropriate treatment, and student’s educational level that could affect success were also assessed.Results: In the present study, 74.3%, 20%, and 5.7% of treatments were successful, failed, and unknown, respectively. Twelve (17.1%) teeth were extracted. The highest success rate was observed in health restoration and it was clinically significant (P=0.076). Age, gender, type of teeth, and jaw also had no significant effect on treatment outcome (P> 0.05). Only a significant association was observed between students’ courses and the success rate of treatment (P <0.05). Also, the highest success rate was seen in proper root canal treatment and health restoration (96.4%), but there was no statistically significant association (P=0.448).Conclusion: The success rate of the 70 endodontic treatments performed by undergraduate students from Mashhad dental school was 74.3%. Proper restoration after root canal therapy is a factor affecting the treatment results.
