تبیین تجارب دستیاران پزشکی از خطا‌های دارویی در بخش مراقبت‌های ویژه نوزادان: مطالعه کیفی

مقدمه: خطاهای دارویی یک رخداد شایع و بالقوه خطرناک برای بیمار است که می تواند در سیستم درمان به عنوان یکی از معیارهای سنجش ایمنی بیمار مورد استفاده قرار گیرد. نوزادان یک جمعیت آسیب پذیر به دلیل تغییر اندازه بدن هستند. استفاده از تجربیات و نظرات افراد در سیستم درمان می تواند منبع مهمی برای جمع آوری اطلاعات و برنامه ریزی در زمینه پیشگیری از خطاهای دارویی باشد. هدف از این مطالعه تبیین تجارب دستیاران پزشکی از خطاهای دارویی در بخش مراقبت های ویژه نوزادان بود.روش : نوع مطالعه حاضر کیفی بود؛ که به روش فنومنولوژی انجام شد. 6 نمونه از بین رزیدنت های بخش ویژه نوزادان یکی از بیمارستان های آموزشی همدان انتخاب شدند. روش جمع آوری داده ها مصاحبه نیمه ساختاریافته بود و تا اشباع داده ها ادامه یافت و تجزیه وتحلیل داده ها، با استفاده از روش کولیزی انجام گرفت.یافته ها: با تحلیل مقایسه ای داده ها 3 طبقه و 9 زیر طبقه تدوین شد. درون مایه های اصلی ناکارآمدی در حرفه ای شدن، نارسایی در فرآیند تجویز دارو توسط پزشکان و ویژگی های فردی و روانی بودند.نتیجه گیری: نتایج نشان داد، نظارت بیشتر بر کار پزشکان کم تجربه در امر مراقبت از بیمار، الکترونیکی کردن پرونده های پزشکی، همت شرکت های داروساز در بسته بندی مناسب داروها و مدیریت کارآمد می تواند در کاهش خطاهای دارویی موثر باشد.

Explanation of Residents' Experiences Concerning Medication Errors in Neonatal Intensive Care Units: A Qualitative Study

Introduction: Medication errors are a potentially hazardous accident for the patients and can be used as a measure of patient safety in the healthcare system. Neonates are the most vulnerable population because of their body size. The experiences and views of those involved in the healthcare system can be a significant source of information gathering and planning in preventing medication errors. This study aimed to explain the residents' experiences concerning medication errors in the neonatal intensive care units. Method: This qualitative study was conducted using phenomenological method in 2017. Six residents were selected from the neonatal intensive care unit of one of the educational hospitals affiliated with Hamedan University of Medical Sciences using Purposive sampling. Data were collocted through semistructured audio recording interviews and interviews were continued until data saturation. Finally, the interviews were completely transcribed and analyzed using Colaizzi's method.Results: With comparative analysis of data, 3 categories or themes and 9 subcategories were developed. The main themes include inefficiency in professionalism, Errors in the process of drug prescription by physicians, and individual and psychological characteristics. Conclusion: According to the results, medication errors can be reduced by more clinical supervision on less experienced physicians in patient care, the use of electronic medical records, and the effort of pharmaceutical companies to package medicines appropriately and have efficient management
