توسعه فرآیند آموزش بالینی اصول و فنون پرستاری با رویکرد مشارکتی - اقدام پژوهی

مقدمه: دانشکده های پرستاری همگام با تغییرات در سیستم های مراقبتی. نیاز به اصلاح در برنامه های آموزش بالینی به ویژه اولین تجربه دانشجو در ورود به بالین دارد. اولین تجربه بالینی به عنوان سنگ بنای حرفه ای در آموزش پرستاری تلقی می شود. مطالعه حاضر با هدف شناسایی چالش های موجود در آموزش بالینی اصول و مهارت های بالینی دانشجویان کارشناسی پرستاری و توسعه آن با رویکرد مشارکتی در دانشکده پرستاری و مامایی دانشگاه علوم پزشکی جندی شاپور اهواز صورت گرفته است. روش کار: مطالعه حاضر، گزارش دو چرخه عمل از یک مطالعه اقدام پژوهی با رویکرد همکاری متقابل و شیوه بحث گروه های متمرکز، و با مشارکت 84 نفر از کلیه افراد ذی نفع می باشد. برای تحلیل داده های حاصل از مصاحبه های گروه های های متمرکز از رویکرد تحلیل محتوا استفاده شد. در چرخه های عمل نیز از روش ارزیابی انعکاسی در حین عمل و همچنین ارزشیابی پایانی در آخر هر چرخه با مشارکت کلیه اعضای درگیر از جمله دانشجویان استفاده شد. نتایج: یافته های مطالعه کیفی نشان داد که، عدم انسجام در برنامه و اجرای برنامه آموزشی؛ مدیریت ناکافی ارتباطات درون و برون سازمانی، عدم درک کافی دانشجو از موقعیت و عدم کنترل مداخلهگرها و عدم استفاده مناسب از تسهیل کننده ها در آموزش و بالین، از چالش های مهم رویاروی روند کارآموزی اصول و مهارت های بالینی در بستر مورد مطالعه می باشد. پس از تعیین مشکلات به صورت طبقه بندی، گروه های صاحب اختیار در تصمیم گیری های آموزشی در دانشکده با همکاری و مشارکت در برنامه کارآموزی اصول و مهارت های بالینی، یک برنامه عملیاتی در چارچوب کلی کوریکولوم درسی مصوب کشوری تدوین نمودند. نتایج ارزشیابی در دو چرخه عمل، موید موفقیت کلی طرح عملیاتی بر مبنای مشارکت حرفه ای مدیران و مربیان از دانشکده و مدیران و پرستاران از بالین به همراه دانشجویان درگیر در همکاری متقابل در آموزش بالینی می باشد. نتیجه گیری: برنامه ریزی با مشارکت کلیه دست اندرکاران آموزشی و بالینی و خود دانشجویان درگیر در کارآموزی اصول و مهارت های بالینی، در کاهش نارسایی های آموزشی و فاصله تئوری و عمل موثر می باشد و رضایتمندی تمام ذی نفعان به دنبال دارد .

Development of Clinical Teaching Process of the Fundamentals of Nursing with Participatory Approach: an Action Research

Introduction: Consistent with health care changes, nursing schools also require revising of clinical education programs, especially, the nursing student rsquo;s first clinical experience. The first clinical experience is considered the cornerstone in professional nursing education. The present study was conducted with the aim to identify challenges of clinical training of the fundamentals of nursing to undergraduate students and develop them with a collaborative approach in the School of Nursing and Midwifery in Ahwaz Jundishapur University of Medical Sciences, Iran. Method: The presents study reports the two operating cycles of an action research with a collaborative approach and focus group discussion, and with the participation of 84 stakeholders. Content analysis approach was used to analyze the data obtained in focus discussion interviews. In addition, reflective evaluation was performed during the operation and final evaluation at the end of each cycle with the participation of all members, including students. Results: The findings of this qualitative study detected a lack of consistency in planning and implementation of curriculum, inadequate intra and extraorganizational communication management, inadequate student understanding of situation, and improper control of restrictors and improper use of facilitators in teaching and practice. After classification of problems, educational decisionmaking authorities of the School developed an operational program within the national curriculum framework through participation in clinical skills and principles training program. Overall, the evaluation results in the two operating cycles confirm that the general success of the action plan is based on professional collaboration of faculty trainers and administrators, and clinical nurses involved in the process. Conclusion: Planning fundamentals of nursing training in partnership with all those involved in practice and education, together with students can be effective in reducing educational failures, gap between theory and practice, and result in the satisfaction of all stakeholders.
