تاثیرجراحی پرینئورافی و ترمیم قدامی خلفی (a p r) روی تصویر بدنی، عملکرد جنسی و کیفیت زندگی زنان

زمینه و هدف: افتادگی یا پرولاپس ارگان‌های لگنی یکی از عوامل افزایش شیوع (female sexual dysfunction) fsd بوده و با اختلال بر عملکرد جنسی بر کیفیت زندگی زنان و همسرانشان و رابطه زناشویی آنان تاثیر می‌گذارد. پژوهش حاضر به بررسی تاثیر پرینئورافی و ترمیم قدامی خلفی واژن روی تصویر بدنی، عملکرد جنسی و کیفیت زندگی زنان پس از عمل پرداخته است.روش بررسی: این تحقیق از نوع کارآزمایی بالینی بدون گروه کنترل و کور نشده است. تعداد 119 نفر از زنان مراجعه‌کننده به بیمارستان‌های شهید بهشتی و الزهرا شهر اصفهان با سنین 49-28 سال و مبتلا به رکتوسل، سیستوسل، پرولاپس رحم و کاندید جراحی پرینئورافی و ترمیم قدامی خلفی واژن با داشتن شرایط ورود به مطالعه از مهر ماه سال 1399تا مرداد ماه سال 1400 وارد مطالعه شدند. ابزار جمع‌آوری اطلاعات سه پرسشنامه عملکرد جنسی زنان (fsfi)، تصویر بدنی (mbsrq) و کیفیت زندگی (sf36) بودکه پیش و شش ماه پس از جراحی تکمیل شدند. تجزیه و تحلیل داده‌ها توسط t وابسته و با کمک نرم‌افزار spss software, version 26 (ibm spss, armonk, ny, usa) انجام گرفت.یافته‌ها: پرینئورافی و ترمیم قدامیخلفی واژن روی تصویر بدنی و بهبود عملکرد جنسی و کیفیت زندگی زنان (0.001>p) تاثیرگذار بوده است.نتیجه‌گیری: به‌نظر می‌رسد جراحی پرینئورافی و ترمیم قدامی خلفی واژن با تاثیرگذاری بر تصویر بدنی بهبود عملکرد جنسی و کیفیت زندگی می‌تواند در ارتقای کیفیت و بهبود روابط زوجینی که نیاز به این عمل جراحی دارند موثر باشد.

The effect of Perineoplasty surgery and anterior-posterior repair (APR) on body image, sexual function, and quality of life in women.

Background: Pelvic organ prolapse is one of the elements increasing the prevalence of Female Sexual Dysfunction which influences the lifestyles of women and their marital relationships. One of the critical desires of surgical remedies of those women, further than enhancing signs and symptoms and restoring regular anatomy, is to preserve and enhance sexual function. The effect of perineorrhaphy and anteriorposterior vaginal repair on body image, sexual function, and quality of life in postoperative women is one of the objectives of this study.Methods: The present study is a clinical trial without a control group. 119 women referred to Shahid Beheshti and AlZahra hospitals in Isfahan, aged 2849 years, with rectocele and cystocele, uterine prolapse and a candidate for perineal surgery and anteriorposterior vaginal repair, entered the study from October 2020 to August 2021. Data collection tools were three questionnaires on Female Sexual Function Index (FSFI), Multidimensional Body -Self Relations Questionnaire (MBSRQ), and ShortForm Health Survey (SF36) that were completed before and 6 months after surgery by interviewing individuals. All surgeries were performed by a Fellowship in pelvic floor disorder, and the type of surgery was the same in all patients. Data were analyzed using a dependent ttest.Results: Data of 119 women with rectocele, second and thirddegree cystocele with maximal grade 1 apex prolapse, and candidates for perineorrhaphy and anteriorposterior vaginal repair were evaluated. The mean age of the study samples was 36.40 ±6.39 years and the age range was 28 to 49 years (before menopause). In the postoperative stage; perineorrhaphy and anteriorposterior vaginal repair affect body image, improvement of sexual function, and quality of life in women (P<0.001). Conclusion: It seems that perineorrhaphy and anteriorposterior vaginal repair affect the body image, Improving sexual function and quality of life, and can be effective in improving the quality and improving the relationship between couples who need this surgery.
