چکیده
|
زمینه و هدف: در بررسی وضعیت بیماران آرتریت روماتویید فاکتورهای التهابی مانند پروتیین واکنشگر c، سرعت رسوب گلبولهای قرمز، میزان فعالیت بیماری و بهویژه نسبت پلاکت به لنفوسیت و نسبت نوتروفیل به لنفوسیت دارای اهمیت میباشند. در این مطالعه نسبت پلاکت به لنفوسیت و نسبت نوتروفیل به لنفوسیت در دو گروه بیماران با و بدون عوارض چشمی مقایسه گردید.روش بررسی: این مطالعه یک بررسی تحلیلیمقطعی است که روی 246 بیمار مبتلا به آرتریت روماتویید مراجعهکننده به کلینیک روماتولوژی کرمانشاه در بازه زمانی آذر 1397 تا اردیبهشت 1398 و با کد اخلاق (ir.kums.rec1397.311) انجام شده است. 191 نفر از بیماران بدون عوارض چشمی و 55 بیمار دچار عوارض چشمی بودند و از نظر سن و جنس همسانسازی شدند. شمارش سلولهای خونی با دستگاه sysmex kx21 hematology analyzer انجام گرفت. نسبت پلاکت به لنفوسیت و نسبت نوتروفیل به لنفوسیت با آزمون همبستگی اسپیرمن و میزان فعالیت بیماری بین دو گروه با mannwhitney u test مقایسه گردید.یافتهها: بررسی نتایج این مطالعه اختلاف معناداری بین سطح نوتروفیل به لنفوسیت و پلاکت به لنفوسیت در دو گروه با و بدون عوارض چشمی نشان نداد. اما میزان فعالیت بیماری در گروه با عوارض چشمی بهطور معناداری پایینتر از گروه بدون عوارض چشمی بود (001/0p<).نتیجهگیری: فاکتورهای التهابی مانند نسبت پلاکت به لنفوسیت و نوتروفیل به لنفوسیت، نمیتوانند به تنهایی در پیشبینی وجود یا احتمال درگیری چشمی مورد قضاوت قرار گیرند، سطح این فاکتورها در بیماران دچار عوارض چشمی تحت تاثیر عوامل دیگری مانند تعداد سلولهای خونی و شرایط هر بیمار قرار میگیرد.

The evaluation of neutrophil to lymphocyte ratio and platelet to lymphocyte ratio in patients with rheumatoid arthritis with and without ocular complications

Abstract
|
Background: Rheumatoid Arthritis patients are evaluated during treatment for various inflammatory factors such as Creactive protein, Erythrocyte Sedimentation Rate, and Disease Activity Score, and other immune systemrelated factors. In the followup of patients with rheumatoid arthritis, hematologic factors associated with the immune system especially Platelet to Lymphocyte Ratio and Neutrophil to Lymphocyte Ratio are important. In this study, platelet to lymphocyte ratio and Neutrophil to lymphocyte ratio were compared in two groups of patients with and without ocular complications.Methods: This crosssectional study was performed on 246 patients with rheumatoid arthritis who were referred to the rheumatology clinic of Kermanshah from December 2018 to May 2019. This study was carried out in accordance with the approval of the ethics committee (IR.KUMS.REC1397.311) at Kermanshah University of Medical Sciences. Of these patients, 191 had no ocular complications and 55 patients had ocular complications and were matched for age and sex. The blood samples were taken from patients and blood cell count was measured by Sysmex KX21 hematology analyzer. The Spearman correlation test was used to evaluate the relationship between platelet to lymphocyte ratio and neutrophil to lymphocyte ratio in both groups of patients without ocular complications and with ocular complications. The Disease Activity Score was compared between the two groups using the MannWhitney test.Results: The results of this study showed no significant difference between NLR and PLR levels in both groups of patients without ocular complications and with ocular complications. But the results showed that DAS28 was significantly lower in the group with ocular complications (P<0.0001).Conclusion: In general, the results of the present study showed that the evaluation of inflammatory factors such as platelet to lymphocyte ratio and neutrophil to lymphocyte alone could not be judged in predicting the presence or possibility of ocular involvement, and the level of these factors in patients with ocular complications was affected. Other factors, such as the number of blood cells and the condition of each patient, are included.
