بررسی مقایسه ای دیدگاه های کارورزان و دستیاران تخصصی سال اول پیرامون علل بروز و افشای خطاهای پزشکی در بیمارستان های آموزشی دانشگاه علوم پزشکی جندی شاپور اهواز در سال 1398 - 139

خطای پزشکی رویداد زیانبار و ناخواسته ای است که به صورت بالقوه میتواند باعث ایجاد عوارض و وقوع مرگ و میر بیماران شود. این مسئله با پایبندی به استانداردهای پزشکی سختگیرانه، می تواند قابل پیشگیری باشد. جمع آوری داده ها از سطح آگاهی کارکنان درمانی می تواند پنجرهای به افزایش سطح استاندارد خدمات و محدود کردن وقوع خطاهای پزشکی باشد.روش بررسی مطالعه حاضر از نوع اپیدمیولوژیک توصیفی – تحلیلی و مقطعی بود که با هدف تعیین دیدگاه 300 نفر ازکارورزان و دستیاران تخصصی سال اول نسبت به علل و اظهار خطاهای پزشکی در بیمارستان های آموزشی دانشگاه علوم پزشکی اهواز در سال 1398 - 1397 انجام شد و داده ها تجزیه و تحلیل شدند.یافته ها میانگین نمره خطای پزشکی در کارورزان به تفکیک حیطه های مورد بررسی در حد متوسط بود. بین دستیاران باکارورزان بر اساس شیوع خطای پزشکی به علت ویژگی های خاص هر بیمار و بیماری رابطه معنادار وجود داشت ) 008 / 0 =p ( . نگرش کارورزان و دستیاران نسبت به علل ارتکاب خطای پزشکی در حد متوسط بود. بین دستیاران با کارورزان بر اساس نگرش کلی به دلایل خطاهای پزشکی رابطه معنادار وجود نداشت ) 05 / 0 < p ).نتیجه گیری بر اساس نتایج به دست آمده از این مطالعه شایعترین علت بروز خطاهای تشخیصی پزشکی مربوط به ویژگیهای خاص هر بیمار و بیماری است که پیشنهاد می شود با ایجاد زیرساختارهای الکترونیکی جهت ثبت سیستمی داده های حاصل از شرح حال و معاینات و ایجاد فرایندی برای در دسترس بودن سابقه پرونده پزشکی گذشته بیمار، میزان بروز خطاها کاهش یابد. همچنین افزایش تعداد نفرات کارکنان پزشکی در کشیکها باید مد نظر قرار گیرد.

a comparative study of the viewpoints of interns and first-year specialist assistants regarding the causes and disclosure of medical errors in the teaching hospitals of jundishapur university of medical sciences, ahvaz, in 2018-2019

background and objectives medical errors are harmful and unintended events that lead to complications, morbidity, and potential consequences. this can be potentially prevented by adhesion to strict medical standards. descriptive data collection for awareness of involved medical staff can enhance the standards of care and restriction for medical error occurrence. subjects and methods the present survey was established as a descriptive-analytical cross-sectional epidemiological study. it aimed to determine the viewpoints of 300 interns and new residents regarding the causes and disclosure of medical errors in teaching hospitals of ahvaz jundishapur university of medical sciences in 2018-2019. the obtained data were analyzed. results the mean prevalence of medical errors in interns in the investigated areas was moderate. there was a significant relationship between assistants and interns based on the prevalence of medical errors due to the specific characteristics of each patient and disease (p=0.008) the attitude of interns and assistants towards the causes of medical errors was moderate. there was no significant relationship between residents and interns based on their general attitudes toward the causes of medical errors (p> 0.05). conclusion as evidenced by the results of this study, the most common cause of medical errors for missed diagnosis is related to the specific characteristics of each patient and the disease. it is suggested to reduce the occurrence of medical errors by creating electronic infrastructures for the systematic recording of data from the history and examinations and creating a process for the availability of the patient’s past medical records. furthermore, increasing the number of medical staff per shift should be noted.
