علل استفاده از روشهای مختلف بیهوشی در سزارین در بیمارستان های آموزشی منتخب واقع در استان خوزستان

زمینه و هدف بیهوشی یکی از خطرهای عمده ی عمل سزارین است و تصمیم برای انتخاب نوع بیهوشی به دلایل مختلفی بستگی دارد. این مطالعه با هدف شناسایی علل استفاده از روش های مختلف بیهوشی در سزارین در بیمارستان های آموزشی منتخب واقع در استان خوزستان انجام شد.روش بررسی پژوهش کیفی حاضر با استفاده از روش تحلیل محتوا انجام گرفت. جامعه ی پژوهش شامل تمام پزشکان وکارشناسان بیهوشی شاغل در دو بیمارستان آموزشی واقع در دو شهرستان بود. اطلاعات لازم از طریق مصاحبه ی نیمه ساختاریافته با روش نمونه گیری هدفمند تا اشباع نظری داده ها به دست آمد. داده ها با استفاده از روش تحلیل محتوای کیفی به صورت قراردادی تجزیه وتحلیل شدند.یافته ها برای انتخاب نوع بیهوشی، دو تم اصلی اولویت از سوی پزشک و اولویت از سوی سازمان به دست آمد. اولویت ازسوی پزشک شامل اولویت نوع خدمت )ترجیح بیمار و ترجیح جراح( و دلایل تخصصی )عوارض کمتر، علمی و تکنیکی( واولویت از سوی سازمان شامل روتین بیمارستان )انتخاب اکثریت( و مدیریت زمان )کنترل بهتر زمان اتاق عمل( می شود.نتیجه گیری روش ترجیحی متخصص بیهوشی برای سلامتی بیمار، علاوه بر علل علمی و تکنیکی، مطابق با اولویت های جراح و بیمار و همچنین، اولویت های مدنظر سازمان تعیین میشود. برپایی دوره های آموزشی برای پزشکان بیهوشی و ایجادبسترهای انتخاب آگاهانه و تصمیم گیری آزاد از طریق ارائه ی اطلاعات کافی و جامع درباره ی عوارض و منافع روش انتخابی برای بیمار ضروری است.

reasons for using different methods of anesthesia in cesarean section in selected teaching hospitals located in khuzestan province

background and objectives anesthesia is one of the major risks of cesarean surgery, and the decision to choose the type of anesthesia depends on various reasons. this study aimed to identify reasons for using different methods of anesthesia in cesarean section in selected teaching hospitals located in khuzestan province. subjects and methods the present qualitative research was conducted using the content analysis method. the research population included all doctors and anesthesiologists working in two teaching hospitals located in two cities. the necessary information was obtained through a semi-structured interview with a purposeful sampling method until the theoretical saturation of the data. the data were analyzed using the qualitative content analysis method. results to choose the type of anesthesia, two main themes of preference from the doctor and preference from the organization were obtained, which preference from the doctor includes the priority of the type of service (patient and surgeon preference) and specialized reasons (fewer side effects, scientific and technical) and priority from the side of the organization, they include hospital routine (majority choice) and time management (better control of operating room time). conclusion the preferred method of the anesthesiologist for the health of the patient is determined in addition to scientific and technical reasons according to the priorities of the surgeon and the patient as well as the desired priorities of the organization. it is necessary to establish training courses for anesthesiologists and create platforms for informed choice by providing about the complications and benefits of the chosen method for the patient.
