خان هفتم در تجویز منطقی دارو: کاربرد اقتصاد رفتاری در طراحی سامانه‌ی نسخه‌نویسی الکترونیک

تجویز منطقی دارو می‌تواند از عوارض و هزینه‌های روزافزون مراقبت‌های سلامت برای بیمار و نظام سلامت پیشگیری کند. به این منظور در کشورهای مختلف مداخلات مدیریتی، قانونی و آموزشی متعددی انجام شده است. این فعالیت‌ها توانسته تجویز غیرمنطقی را تا حدودی کنترل کند. اقتصاد رفتاری تلاش می‌کند بدون استفاده از اجبار افراد را به تغییر رفتار ترغیب کند. مهم‌ترین ابزار این شاخه‌ی جدید از علم اقتصاد، شناسایی خطاهای شناختی قابل پیش‌بینی و تلاش برای کاهش آن‌هاست. مداخلات مختلفی در دنیا برای تجویز منطقی دارو با استفاده از اقتصاد رفتاری انجام می‌شود.مطالعه‌ی حاضر چهار مورد از این مداخلات شامل تغییر گزینه‌ی پیش‌فرض، اثر هم گروهی، گزینه‌های پیشنهادی و توجیه پاسخگو را مرور می‌کند. به نظر می‌رسد با توجه به گسترگی استفاده از سامانه‌های نسخه نویسی الکترونیک استفاده از این روش‌ها می‌تواند به بهبود وضعیت تجویز منطقی دارو در کشور منجر شود. اگر چه مداخلات اقتصاد رفتاری نمی‌تواند جایگزینی برای آموزش و قانون‌گذاری و سایر فعالیت‌های کنونی باشد اما می‌تواند به عنوان مکملی برای تغییر رفتار مورد توجه قرار گیرد.

the last mile in rational prescription: the application of behavioral economics in the design of the electronic prescribing system

rational prescription of medications can prevent complications and decrease costs for healthcare systems and patients. for this purpose, many administrative, legal, and educational interventions have been implemented in different countries. these initiatives have been able to control the irrational prescription to some extent. behavioral economics tries to persuade people to change their behavior without using coercion. the most important tool of this new branch of economics is identifying predictable cognitive biases and trying to reduce their effect. various interventions are carried out in the world for the rational prescription of drugs using behavioral economics. the current study reviews four interventions: changing the default option, the peer group effect, suggested alternatives, and accountable justification. it seems that due to the widespread use of electronic prescribing systems, these methods can lead to the improvement of the rational prescription of drugs in the country. although behavioral economics interventions are not substitutes for education, legislation, and other current activities, they can be considered a complement to behavior change.
