بررسی یافته‌های پاپ اسمیر و کولپوسکوپی در بیماران hpv مثبت

مقدمه: مطالعه‌ی حاضر با هدف تعیین یافته‌های کولپوسکوپی و هیستوپاتولوژی در بیماران hpv مثبت به گونه high real tech  انجام شد.روش‌ها: در این مطالعه مقطعی 100 زن hpv مثبت مراجعه کننده به کلینیک انکولوژی دانشگاه علوم پزشکی شهرکرد در سال 1400 که قبلاً تست پاپ اسمیر انجام داده‌اند، تحت کولپوسکوپی قرار گرفته و فراوانی ضایعات سرویکس در دو روش مذکور تعیین و میزان توافق دو روش مورد ارزیابی قرار گرفت.یافته‌ها: از بین 100 بیمار (human papilloma virus) hpv مثبت، در 34 نفر (34 درصد) یافته‌های پاپ اسمیر و کولپوسکوپی هر دو نرمال بوده و در 35 نفر (35 درصد) یافته‌های پاپ اسمیر و کولپوسکوپی هر دو غیر نرمال بود. در مقابل، در 17 بیمار (17 درصد) یافته‌های پاپ اسمیر غیرنرمال ولی یافته‌های کولپوسکوپی نرمال بود. همچنین در 14 بیمار (14 درصد) یافته‌های پاپ‌اسمیر نرمال ولی یافته‌های کولپوسکوپی غیرنرمال بود. برابر آزمون کاپا، میزان توافق بین پاپ‌اسمیر و کولپوسکوپی در تشخیص ضایعات نرمال و غیرنرمال، 0/38 بود که از نظر آماری معنی‌دار بود (0/001 > p).نتیجه‌گیری: یافته‌های مطالعه‌ی حاضر نشان داد، میزان توافق قابل قبولی بین پاپ‌اسمیر و کولپوسکوپی در تشخیص ضایعات سرویکس در بیماران hpv مثبت وجود دارد. از طرف دیگر امکان تشخیص ضایعات با درجه بالا در کولپوسکوپی محتمل‌تر می‌باشد.

background: the present study aimed to determine colposcopy and histopathology findings in hpv positive patients as high real tech done.methods: in this cross-sectional study, 100 hpv-positive women referred to the oncology clinic of shahrekord university of medical sciences in 2021-2022, who had previously undergone a pap smear test, underwent colposcopy, and the frequency of cervical lesions detected by the two mentioned methods was determined and the degree of agreement between the two methods was evaluated.findings: out of 100 hpv positive patients, both pap smear and colposcopy findings were normal in 34 (34%) patients and both pap smear and colposcopy findings were abnormal in 35 (35%) patients. on the other hand, in 17 patients (17%) pap smear findings were abnormal but colposcopy findings were normal. also, in 14 patients (14%), pap smear findings were normal, but colposcopy findings were abnormal. according to the kappa test, the agreement between pap smear and colposcopy in the diagnosis of normal and abnormal lesions was 0.38, which was statistically significant (p < 0.001).conclusion: the findings of the present study showed that there is an acceptable level of agreement between pap smear and colposcopy in the diagnosis of cervical lesions in hpv positive patients. on the other hand, detection of high-grade lesions is more likely with colposcopy.
