بررسی عوارض و موفقیت عمل جراحی pcnl در مبتلایان به سنگ کلیه در کلیه پیوندی

مقدمه: پیوند کلیه به عنوان درمان طولانی‌مدت اصلی در بیماران مبتلا به نارسایی مزمن کلیه است که موجب امید به زندگی بیشتر بیماران می‌شود؛ با این حال عوارض و موفقیت عمل بررسی در این زمینه را مهم می‌نماید. در این راستا مطالعه با عنوان تعیین عوارض و موفقیت عمل جراحی pcnl در مبتلایان به سنگ کلیه در کلیه پیوندی انجام شد.روش‌ها: این مطالعه‌ی مقطعی گذشته‌نگر بر روی کلیه بیماران مراجعه‌کننده به مرکز درمانی الزهرا(س) اصفهان با تشخیص سنگ در کلیه یا حالب پیوندی که همگی تحت جراحی pcnl قرار گرفتند در سال‌های 1388-1400 انجام شد. مطالعه بر روی عوارض از جمله نیاز به تزریق خون و افزایش میزان کراتینین بیماران، مقایسه با پیش از جراحی و درصد پاکسازی سنگ به طور کامل بود. داده‌ها با آزمون chi-square تحلیل شد.یافته‌ها: نتیجه‌ی مطالعه پاکسازی 100 درصد تمام بیماران از سنگ را نشان داد. همچنین بین میزان کراتینین قبل و بعد از عمل تفاوت معنی‌دار مشاهده نشد. بیماران دچار هیچ عارضه‌ای نشده و تنها یک مورد تب و یک مورد تزریق خون پیش از جراحی گزارش شد.نتیجه‌گیری: یافته‌های مطالعه نشان‌دهنده‌ی موفقیت pcnl در پاکسازی کامل سنگ در این بیماران و عوارض حداقلی آن بود. البته با توجه به محدود بودن تعداد نمونه‌ها و پیگیری‌های پس از عمل، به مطالعات بیشتر با مدت زمان و تعداد نمونه‌ی کافی نیاز است.

investigating complications and success of pcnl surgery in patients with kidney stones in transplanted kidney

background: kidney transplantation is the main long-term treatment for patients with chronic kidney failure, which increases the life expectancy of patients. however, the complications and the success of investigations in this field are important. in this regard, this study was conducted to determine the complications and success rate of pcnl surgery in patients with kidney stones in transplanted kidneys.methods: this retrospective cross-sectional study was conducted on all patients referred to al-zahra medical center in isfahan who were diagnosed with stones in a transplanted kidney or ureter and underwent pcnl surgery between 2009 and 2021. the study focused on complications, including the need for blood transfusions and increased creatinine levels of patients compared to pre-surgery values, as well as the rate of complete stone clearance. the data were analyzed with the chi-square test.findings: the results of the study showed that 100% of all patients were free of stones. also, no significant difference was observed between creatinine levels before and after the operation. the patients did not have any complications; only one case of fever and one case of blood transfusion before surgery was reported.conclusion: the findings indicate that pcnl is highly successful in achieving complete stone clearance in patients with transplanted kidneys, with minimal associated complications. however, due to the limited sample size and post-operative follow-up, further studies with a larger sample size and longer duration are recommended.
