دو اختلال فریبنده‌ی سیستم اوروروژنیتال با علایم مشابه و درمان‌های متفاوت

مقدمه: ریفلاکس پیشابراه به واژن، یک اختلال عملکردی در دفع ادرار است که طی آن، در هنگام ادرار کردن، ادرار به داخل واژن برمی‌گردد، و می‌تواند باعث vulvovaginitis، لیک ادرار از واژن و بوی نامطبوع ادرار شود.روش‌ها: بیمار، یک دختر 12 ساله مبتلا به مثانه نوروژنیک در زمینه‌ی seizure، که با شکایت قطره قطره آمدن ادرار پس از ادرار کردن و بی‌اختیاری ادرار بدون حس نیاز به دفع ادرار (post-void dribbling and unaware urinary incontinence)، شب ادراری مراجعه و تحت بررسی قرار گرفته و بی‌اختیاری ادرار در زمینه فیستول واژن به مثانه مطرح گردیده بود، اما پس از انجام مطالعات تشخیصی، جهت انجام ارزیابی‌های بیشتر به مرکز ما ارجاع داده شده بود.یافته‌ها: معاینه‌ی ژنیتالیا طبیعی بود، اما با ایستادن و راه رفتن نشت ادرار به وضوح قابل مشاهده بود. بیمار تحت ارزیابی با سونوگرافی، vcug و تست یورو داینامیک قرار گرفت. بر اساس نتایج معاینات و تست‌های انجام شده، برای بیمار تشخیص urethrovaginal reflux داده شد و بیمار تحت درمان مناسب قرار گرفت.نتیجه‌گیری: شایع‌ترین علت بی‌اختیاری ادرار در کودکان، اختلالات عملکردی می‌باشد. ریفلاکس یورتروواژینال، یکی از علت‌های بی‌اختیاری پاتولوژیک و یا فیریولوژیک در دخترها است و عمدتاً در سنین قبل از بلوغ دیده می‌شود. این اختلال می‌تواند در زمینه‌ی اختلالات آناتومیکال مانند پاراواژینال وب و یا چسبندگی لابیاها ایجاد شود. تشخیص این اختلال عمدتاً نیاز به ارزیابی‌های تهاجمی ندارد، گرچه در صورت همراهی با اختلالات عملکردی، الگو و شدت بی‌اختیاری ادرار تغییر و تشخیص صحیح نیازمند ارزیابی‌های بیشتر خواهد بود.

two deceptive genitourinary system disorders with similar symptoms but different treatments: urethrovaginal reflux and vesicovaginal fistula

background: urethrovaginal reflux is an uncommon cause of voiding difficulties in girls, characterized by the backward filling of the vagina during urination. symptoms may include vulvovaginitis, vaginal discharge, and an unpleasant odor, which can arise from both pathological and physiological factors contributing to urinary incontinence in young girls.methods: in one case, a 12-year-old girl with a history of neurogenic bladder due to seizures presented with post-void dribbling and unaware urinary incontinence, enuresis, and recurrent symptomatic urinary tract infections (utis). initially, the possibility of a vesicovaginal fistula was considered. however, both cystoscopy and a dye test returned normal results. the patient was then referred to our center for further evaluation.findings: a thorough physical examination revealed normal findings. however, urinary leakage from the vagina was observed when the patient was standing or walking. abdominal and pelvic sonography indicated that the size and cortical thickness of the kidneys were normal, though the bladder wall thickness was increased. following vcug and urodynamic tests, urethrovaginal reflux was diagnosed, and the patient received appropriate treatment.conclusion: dysfunctional voiding is the most prevalent cause of urinary incontinence in children, with urethrovaginal reflux being a common factor in prepubertal girls. this type of reflux can be seen across anatomical anomalies such as paraaginal web and/or labial adhesions. in most patients, the diagnosis of urethrovaginal reflux is determined through medical history, physical examination, and a bladder diary. however, if accompanied by an overactive bladder or neurogenic bladder, the pattern and severity of urinary incontinence may vary, necessitating further evaluation for accurate diagnosis.
