مقایسه‌ی اثر بخشی تزریق داخل ضایعه‌ای گلوکانتیم در درمان لیشمانیوز جلدی با مزوتراپی و درمان با روش معمول

مقدمه: ایران یکی از مناطق هیپرآندمیک لیشمانیوز جلدی می‌باشد. درمان مطلوب برای این بیماری هنوز ناشناخته است. مزوتراپی تکنیک درمانی خاصی است که می‌تواند انواع مختلف داروها را با استفاده از تزریقات متعدد به لایه‌های زیر پوست، در عمق دلخواه تزریق کند.روش‌ها: در یک کارآزمایی بالینی، 90 نفر از بیمارانی که با تشخیص قطعی لیشمانیوز جلدی به مرکز تحقیقات سالک اصفهان مراجعه کرده بودند، انتخاب و به صورت تصادفی به 2 گروه مساوی تقسیم شدند. گروه اول با تزریق داخل ضایعه به روش معمول و گروه دوم با روش مزوتراپی درمان شدند. در هر دو گروه تزریق داخل ضایعه‌ی گلوکانتیم هفته‌ای یک بار تا بهبود ضایعه و در نهایت تا 6 هفته انجام شد. بیماران طی درمان، یک هفته بعد، یک ماه بعد و 3 ماه بعد پی‌گیری گردیدند. 0.05 > p به عنوان سطح معنی‌دار بودن تلقی گردید.یافته‌ها: 60 بیمار مطالعه را ادامه دادند. میانگین تغییرات شاخص‌های بیماری (اندازه زخم، اریتم و ایندوراسیون) در گروه معمول با گروه مزوتراپی تفاوت آماری معنی‌دار نداشت. سرعت بهبود ضایعات بر اساس سرعت بهبود زخم، اریتم و ایندوراسیون در گروه مزوتراپی از گروه معمول به صورت معنی‌داری کمتر بود. میانگین شدت درد و میانگین حجم مورد نیاز دارو به ازای هر سانتی‌متر مربع ضایعه در روش مزوتراپی (به ترتیب 0.06 و 1.05) از روش معمول (به ترتیب 7.67 و 1.6) به طور معنی‌داری کمتر بود ولی بین میزان بهبودی بالینی و آزمایشگاهی تفاوت معنی‌داری وجود نداشت.نتیجه گیری: با وجود آن که دو گروه از لحاظ میانگین تغییرات شاخص‌های بیماری و میزان بهبود کلینیکی و آزمایشگاهی تفاوتی نداشتند، تزریق با استفاده از مزوتراپی سریع‌تر و ارزان‌تر بوده، با درد کمتری همراه است و باعث افزایش همکاری بیماران در پروسه‌ی درمان می‌شود.

The Efficacy of Intralesional Injection of Glucantime with Mesotherapy for the Treatment of Cutanuos Leishmaniasis in Comparison to Classic Method

<ul><li><div class=abstract><strong>Background:</strong> Cutaneuos leishmaniasis is a common parasitic disease in 88 countries. Iran is one of the hyperendemic regions of cutaneuos lieshmaniasis. Mesotherapy is a technique used to deliver medication to a specific area of the body to treat a specific condition. This is done by painless multiple intradermal microinjections.</div></li><li><div class=abstract><strong>Methods: </strong>In this clinical trial study, 90 patients with confirmed cutaneuos lieshmaniasis in Isfahan Lieshmaniasis Research Center, were selected and randomly divided into 2 groups. Group A and B were treated by classic intralesional and mesotherapy injection respectively; intralesional injection of glucantime once a week for a maximum of 6 weeks was done for both groups. Patients were followed one week, one month and 3 months after the treatment. P value < 0.05 was considered as significant level.</div></li><li><div class=abstract><strong>Results:</strong> Sixty patients completed the study. There was no significant difference between the mean of changes of disease indecies of the lesions (size of ulcer, erythema and induration) in classic group and mesotherapy group. The speed of the lesioncure regarding cure of ulcer, erythema and induration in the mesotherapy group was significantly less than classic group. The mean value of pain severity and the mean volume of the drug needed for each centimeter of the lesions in Mesotherapy group was significantly less than classic group. The results showed no statistical significant difference between the clinical and parasitological cure rate of the two groups.</div></li><li><div class=abstract><strong>Conclusion:</strong> Intralesional injection of glucantime by mesutherapy was more rapid, painless and cost effective than the classic method. This could promote the compliance of patients in the treatments procedure.</div></li></ul>
