مقایسه حساسیت و اختصاصیت تست بروسلا کپچر با تست استاندارد آگلوتیناسیون لوله ای رایت در تشخیص بیماری بروسلا

زمینه و هدف: بروسلوز یکی از بیماری های شایع عفونی مشترک بین انسان و دام در ایران است. به دلیل مشکلات تشخیصی بروسلوز به ویژه در مناطق اندمیک استفاده از روش های تشخیصی جدید از اهمیت بالایی برخوردار است. از این رو این مطالعه با هدف بررسی مقایسه حساسیت و اختصاصیت تست بروسلا کپچر با تست استاندارد آگلوتیناسیون لوله ای رایت در تشخیص بیماری بروسلا انجام شد. مواد و روش ها: در این مطالعه مقطعی از ابتدا تا انتهای سال 1399 در مجموع 100 سرم از افرادی که دارای علائم مشکوک به بیماری بروسلوز و فاقد تایید آزمایشگاهی بودند مورد مطالعه قرار گرفت. آزمون رایت (wright)، کومبس رایت  coombs wright)), بروسلا کپچر(brucellacapt) در تیترهای آستانه تشخیصی160/1 و 80/1  انجام شد. حساسیت، ویژگی، ارزش اخباری مثبت و منفی بررسی شد. سپس ضریب همبستگی نتایج در تشخیص بیماری با تست توافقی کاپا در نرم افزار ((spss ورژن 14 تجزیه و تحلیل گردید.یافته‌ها: در مجموع 100 بیمار وارد مطالعه شد که 45 درصد زن و 55 درصد مرد بودند. در تیتر160/1 آزمون رایت مثبت به عنوان آستانه تشخیصی، حساسیت، ویژگی و مقادیر پیش بینی مثبت و منفی برای آزمون brucellacapt به ترتیب درصدهای97/95، 86/27 ،87/27  و 97/77بود و در تیتر 80/1  تست رایت مثبت به عنوان آستانه تشخیصی حساسیت، ویژگی و مقادیر پیش بینی مثبت و منفی آزمون brucellacapt به ترتیب درصدهای 98/43، 97/50، 95/45 و97/05 بود. نتیجه‌گیری: یافته ها نشان داد که تست brucellacapt در نقاط برش مورد مطالعه حساسیت، ویژگی، ارزش اخباری مثبت و منفی قابل قبولی جهت تشخیص بیماری بروسلوز دارد و قدرت تشخیصی و دقت این تست در تیتر 160/1 نیز به تست coombs wright بسیار نزدیک است.

comparison of brucellacapt sensitivity and specificity with standard wright tube agglutination test in the diagnosis of brucella

background and aim: brucellosis is one of the most common infectious diseases between humans and livestock in iran. because of the diagnostic problems of brucellosis, especially in endemic areas, the use of new diagnostic methods is of great importance. therefore, this study was performed to compare brucellacapt sensitivity and specificity with the standard wright tube agglutination test in the diagnosis of brucella. materials and methods: a total of 100 sera from a patient with suspected symptoms of brucellosis and without laboratory confirmation were studied. wright, coombs wright, and brucellacapt tests were performed at diagnostic threshold titers of 1.160 and 1.80 sensitivity, specificity, and positive and negative predictive values were evaluated and finally, data were analyzed with spss software version 14. results: a total of 100 patients were included in the study, of which 45% were female and 55% were male. in titrate 1.160 positive wright test as a diagnostic threshold, sensitivity, specificity, and positive and negative predictive values for the brucellacapt test were 97.95, 86.27, 87.27, and 97.97 percent, respectively. in titrate 1.80 positive wright test as a diagnostic threshold, sensitivity, specificity, and positive and negative predictive values of brucellacapt test were 98.43, 97.50, 95.45, and 97.05 percent, respectively. conclusion: the results showed that the brucellacapt test in the studied cut points has acceptable sensitivity, specificity, and positive and negative predictive value for the diagnosis of brucellosis and the diagnostic and accuracy of this test in the titrate 1.160 is very close to the coombs wright test.
