تاثیر برنامه مراقبت مبتنی بر الگوی سازگاری روی در بعد فیزیولوژیک بر سطح هوشیاری بیماران در بخش مراقبت ویژه: یک مطالعه کارآزمایی بالینی

زمینه و هدف: یکی از شاخص های بهبودی در بخش مراقبت ویژه ارتقاء سطح هوشیاری بیماران می باشد. عوامل مختلفی از جمله نوع مراقبت در تغییرات سطح هوشیاری بیماران موثر هستند. لذا این مطالعه با هدف تعیین تاثیر برنامه مراقبت مبتنی بر الگوی سازگاری روی در بعد فیزیولوژیک بر سطح هوشیاری بیماران بخش مراقبت ویژه انجام شد.روش بررسی: این مطالعه کارازمایی بالینی سه سو کور در سال 1395 بر روی 100 بیمار بخش مراقبت ویژه انجام شد. به صورت تصادفی دو بخش آی سی یو در بیمارستان بعثت سنندج به عنوان بخش مداخله و کنترل انتخاب شدند. بیماران واجد شرایط در هر بخش به صورت مجزا به روش تصادفی ساده به گروه مداخله یا کنترل تخصیص داده شدند. در گروه مداخله مراقبت مبتنی برالگوی سازگاری و در گروه کنترل مراقبت های روتین انجام شد. سطح هوشیاری بیماران بر اساس gcs به مدت 7 روز، روزی 2 بار برای هر بیمار اندازه گیری شد. داده های بدست آمده با نرم افزار spss نسخه21 تحلیل شد. یافته ها: میانگین سطح هوشیاری در زمان های اول تا سوم دارای تفاوت معنی داری بین گروه کنترل و مداخله را نشان نداد (0.05>p). تفاوت میانگین سطح هوشیاری اندازه گیری شده در روز و شب چهارم به بعد در دو گروه کنترل و مداخله تفاوت معنی دار بود(0.05>p).نتیجه گیری: مراقبت مبتنی بر الگوی سازگاری روی توسط پرستاران بعد از سه روز بر افزایش سطح هوشیاری بیماران بخش مراقبت ویژه تاثیر داشت. پیشنهاد می گردد در جهت ارتقاء سلامت بیماران در بخش مراقبت ویژه مراقبت های پرستاری سازمان یافته مبتنی بر الگو های پرستاری ارائه گردد.

Effect of “Roy adaptation model” in physiologic aspect on the level of consciousness of patients in intensive care unit: A clinical trial

Background and Aim: One of the indicators of recovery in the patients admitted to intensive care units is improvement of the level of consciousness. Various factors, including the type of care are effective in the changes of consciousness level of the patients. This study was conducted to determine the effect of nursing care plan based on ldquo;Roy adaptation model rdquo; in physiologic aspect on the level of consciousness of the patients in the intensive care unit.Material and Methods: This tripleblind clinical trial included 100 patients in the intensive care unit in 2017. Two ICU units in Sanandaj hospitals were selected randomly as intervention and control groups. Eligible patients in each unit were randomly assigned to intervention and control groups. The intervention group received nursing care plan based on Roy adaptation model rdquo; and the control group received routine care. We measured level of consciousness for every patient based on the GCS for 7 days, twice a day. We used SPSS version 21 for data analysis.Results: The mean level of consciousness in the first three days and nights did not show any significant difference between the intervention and control groups. But there was a significant difference between the mean levels of consciousness from the fourth day and night on, between intervention and control groups (p <0.05).Conclusion: Roy adaptation model was effective in increasing the level of consciousness after three days in the patients in the intensive care unit. Therefore, we recommend organized nursing care based on standard nursing models for all of the patients in the intensive care units.Keywords: Level of consciousness, Roy adaptation model, Intensive care unit.
