بررسی تاثیر فتوتراپی بر میزان پلاکت در نوزادان مبتلا به زردی بستری شده در بیمارستان شهید رجایی تنکابن از سال 1392 تا 1393

زمینه و هدف : فتوتراپی درسطح وسیعی جهت درمان زردی نوزادی استفاده می شود، درنتیجه عوارض آن اهمیت بسزایی دارد؛ وکمبود پلاکت سبب خونریزی وآسیب مغزی جدی می شود، نیازبه بررسی بیشتر اثر فتوتراپی برسطح پلاکت دیده می شد.روش کار: پژوهش حاضراز نوع تحلیلی مقطعی آیندنگر است که در بخش نوزادان بیمارستان شهید رجایی تنکابن درسال 1393-1392 انجام و در آن 100 نوزاد مبتلابه زردی مورد مطالعه قرار گرفتند. قبل از شروع فتوتراپی درابتدای بستری پلاکت نوزادان شمارش و نوزادانی که پلاکت کمتر از 150000mm3 داشتند از بررسی حذف شدند. بعداز اتمام فتوتراپی پلاکت بررسی گردید. نوزادانی که نیازبه تعویض خون، دچار عفونت، ناسازگاری aborh و بیماری دیگری شدند از مطالعه خارج گردیدند. اطلاعات هر نوزاد پس از جمع آوری در چک لیست، توسط نرم افزار spss18 تجزیه و تحلیل آماری شد. یافته ها: از 100 نوزاد مورد مطالعه 61 نوزاد بعداز دریافت فتوتراپی دچار افت میزان پلاکت شدند، ازاین تعداد 9 مورد دارای پلاکت زیر 150 هزار بودند. میانگین میزان پلاکت قبل فتوتراپی 233400 با انحراف استاندارد 55621.19 و میانگین میزان پلاکت بعد فتوتراپی 228420 با انحراف استاندارد67083.27 بود که بین میزان پلاکت قبل وبعد فتوتراپی ارتباط معناداری وجود نداشت. میانگین درصدکاهش 12.3% با انحراف استاندارد 8.25 ومیانگین درصدافزایش 15.2 % با انحراف استاندارد 17.93 بود. کاهش میزان پلاکت در جریان فتوتراپی با میزان پلاکت در نوزادان ترمی که قبل از فتوتراپی پلاکت آنها درسطح 250-300 هزار بوده با مدت فتوتراپی، وزن، بیلیروبین توتال، سن حاملگی و شدت فتوتراپی ارتباط داشته است.نتیجه گیری: فتوتراپی موجب کاهش شمارش پلاکتی می شود ولی این کاهش ناچیز و در محدوده نرمال بوده است.

Effect Of Phototherapy On Platelet Level In Newborns Admitted To The Tonekabon Shahid Rajai Hospital 2013-2014

Background: phototherapy is broadly used for treatment ichter, therefore phototherapy and its side effects are important, platelet depletion can to bleeding and serious complication, so it is necessary to study carefully the phototherapy effect on platelet level.Methods: this research is an Descriptivecross sectional and prospective study in Shahid Rajaee Hospital, Tonekabon, during 20132014.The number of infants investigated are 100ichteric neonate , Before start of phototherapy, the infants rsquo; platelets were measured by a counter, however the neonates with less than 150000mm3 platelets were eliminated. after Phototherapy was done, the number of platelets is checked. The infants, who needed blood exchange, sepsis, ABORh incompatibility and other sorts of illnesses, were eliminated.information was recorded in checklist, they analysed by spss18 statistically.Results: This study showed that among 100infants understudied, 61of them ranged platelet reduction after phototherapy, that 9neonates in this group showed the platelet level less than 150000.the average platelet levels before phototherapy was 233400with S.D 55621.19and the average of platelet levels after phototherapy was 228420with S.D 167083.27.there wasn rsquo;t a significant relation between platelet levels before phototherapy and after it.this reduction revealed a significant relation toward the amount of platelet among term infants whom their platelet levels were 250000300000 before phototherapy, which related to phototherapy duration, weight, total bilirubin, gestational age and phototherapy intense.Conclusion: phototherapy cause the platelet level reduction, although the platelet reduction was in Normal range.
