معرفی یک مورد حاملگی موفقیت‌آمیز در بیمار مبتلا به آرتریت تاکایاسو

آرتریت تاکایاسو یا به‌اصطلاح بیماری بدون نبض یک بیماری بسیار نادر در جوانان است که علائم اولیه آن در سن 5 تا 40 سالگی ایجاد می‌شود. اگرچه ممکن است هر دو جنس تحت تاثیر قرار بگیرند، 80-90 درصد بیماران زن هستند. این بیماری، آرتریت التهابی مزمنی است که عروق بزرگ را درگیر می‌کند. این شکل آرتریت می‌تواند با آنژیوگرافی غیرطبیعی بخش فوقانی آئورت و شاخه‌های اصلی آن همراه باشد که سبب اختلال در خون‌رسانی عروق اندام فوقانی گردد. حاملگی همراه با سابقه آرتریت تاکایاسو جز حاملگی‌های پرخطر محسوب می‌شود و بسته به پیشرفت بیماری و میزان آسیب به عروق، احتمال خطر افزایش می‌یابد. هدف از این بررسی، گزارش یک مورد حاملگی موفقیت‌آمیز و بدون عارضه در بیمار مبتلا به آرتریت تاکایاسو است. معرفی مورد: بیمار خانم 36 ساله حاملگی سوم صاحب دو فرزند سالم با سن حاملگی 38 هفته که با دردهای زایمانی مراجعه کرده بود. در سابقه آرتریت تاکایاسواز 8 سال قبل داشته که در بارداری فعلی هیچ‌گونه پیگیری انجام نداده و درمانی دریافت نکرده بود. اولین مراجعه وی در 38 هفتگی به علت شروع دردهای زایمانی بود. برای بیمار بررسی‌ها و اقدامات اورژانسی انجام شد و درنهایت به دلیل ارست دیلاتاسیون سزارین اورژانسی انجام گرفت که حاصل این بارداری نوزاد پسر سالم با اپگار 9-10 و وزن 4300 گرم بود.

successful pregnancy in a patient with takayasu’s arteritis: a case report

takayasu arteritis, a so-called pulseless disease, is a very rare disease in young people. the first symptoms appear at the age of 5 to 40 years. although both sexes may be affected, 80-90% of patients are female. it is a chronic inflammatory arthritis that affects the large arteries. this form of arthritis can be associated with abnormal angiography of the upper aorta and its main branches, which disrupts blood flow to the arteries of the upper extremities. pregnancy with a history of takayasu arteritis is considered a high-risk pregnancy and the risk increases depending on the progression of the disease and the extent of vascular damage. the purpose of this article is to report the case of a successful and uncomplicated pregnancy in a patient with takayasu arteritis. a 36-year-old woman in her third pregnancy with two healthy children, with a gestational age of 38 weeks was referred to us with labor pains. the patient had a 8-year history of takayasu’s arteritis. she did not follow-up and received no treatment during her current pregnancy. her first visit in week 38 was due to the onset of labor pains. emergency evaluation and procedures were performed. finally, due to arrest dilatation, an emergency cesarean section was performed. she delivered a healthy boy weighing 4300 g with an apgar score of 9-10.
