Abstract
artificial intelligence (ai) holds significant potential to transform cardiovascular care, yet without explicit attention to fairness, it risks amplifying existing health disparities. this study aims to design and introduce a systematic framework for the equitable development and deployment of ai models in the context of cardiovascular diseases in iran. the proposed framework, termed cardio-fair, is structured as a protocol-oriented methodology encompassing four core phases: (1) data preparation and definition of context-specific socioeconomic indicators as fairness criteria; (2) model development with the integration of fairness considerations into the training process; (3) quantitative evaluation of both performance and fairness using standardized metrics; and (4) implementation, continuous monitoring, and governance. the framework emphasizes transparency, reproducibility, and a balanced integration of technical, clinical, and ethical dimensions. by offering practical strategies for handling sensitive data, subgroup evaluation, ongoing surveillance, and governance structures, cardio-fair bridges the gap between theoretical concepts of fairness and their clinical application. its primary innovations include the localization of fairness indicators, coverage of the entire model lifecycle, and an operational focus. as a comprehensive roadmap, cardio-fair lays the foundation for responsible and equitable ai development in cardiovascular medicine in iran. its success, however, depends on stakeholder commitment and rigorous validation through real-world clinical studies.
