   بررسی تجربه زیسته دانشجویان دارای افسردگی و با سابقه خودآسیبرسانی از اثربخشی درمان ترکیبی هیجانمدار و اصلاح سبک زندگی بر تفکر ارجاعی و خشم بالینی (یک مطالعه کیفی)

چکیده    پژوهش حاضر با هدف بررسی تجربه زیسته دانشجویان دارای افسردگی و افکار و رفتار خود آسیب رسان با قصد خودکشی از درمان ترکیبی هیجان مدار و اصلاح سبک زندگی با تمرکز بر تفکر ارجاعی و خشم بالینی انجام شده است. طرح پژوهش، کیفی و مطالعه چند موردی به روش پدیدارشناسی تفسیری و جامعه پژوهش شامل دانشجویان مراجعه کننده به مرکز مشاوره دانشجویی دانشگاه آزاد اسلامی واحد ساوه ( 1403-1402) بود. نمونه شامل 4 دانشجو با تشخیص افسردگی و دارای افکار و رفتار خود آسیب رسان با قصد خودکشی، نمرات بالا در تفکر ارجاعی و خشم بالینی بوده اند که با روش نمونه گیری هدفمند انتخاب شده اند. ابزارهای پژوهش شامل پرسشنامه های (افسردگی beck, 1996)، تفکر ارجاعی (ehring,2010)، خشم بالینی ((snell et. al,1995 و مصاحبه نیمه ساختاریافته بود. پس از اجرای 12 جلسه درمان ترکیبی هیجان مدار و اصلاح سبک زندگی، مصاحبه ها با روش دیکلمن و همکاران (1989) تحلیل شد. مضامین حاکی از کاهش تفکر ارجاعی و خشم بالینی بوده است. همچنین دو مضمون اصلی (تجارب درمانبخش و عوامل بازدارنده) و هشت زیر مضمون استخراج شد. مضامین تجارب درمانبخش برای تفکر ارجاعی، ادراک کنترل بر روی رفتار در مقابل افکار منفی و تمرکز بر زمان حال و مضمون بازدارنده شامل عوامل محیطی و به طور خاص دوستان و خانواده بود.همچنین مضامین تجارب درمانبخش برای خشم بالینی، شامل پذیرش شرم و غم، عدم اجتناب رفتاری و درک جدیدی از ناکامی ها و رویدادهای ناخوشایند و مضامین بازدارنده نیز، دشوار بودن انتقال یادگیری به بیرون از جلسات درمان و انتظارات دیگران برای بهبودی سریع بود.

Abstract    the study aims to explore the lived experiences of students with depression and suicidal self-injury thoughts and behaviors of combination of emotion-focused therapy and lifestyle modification, with a focus on repetitive negative thinking and clinical anger. this qualitative research used an interpretive phenomenological approach and a multiple case study design. the research population included students visiting the counseling center at the islamic azad university, saveh branch, during the 2023-2024 academic year. the sample included four purposively selected students with depression and suicidal self-injury thoughts and behaviors who scored high on referential thinking and clinical anger. the research tools entailed the beck depression inventory (1996), the repetitive negative thinking scale (ehring, 2010), the clinical anger scale (snell et al., 1995), and semi-structured interviews. after 12 sessions of combined emotion-focused therapy and lifestyle modification, the interviews were analyzed using diekelmann et al.’s method (1989). themes indicated a reduction in repetitive negative thinking and clinical anger. two core categories (therapeutic experiences and inhibitory factors) and eight sub-categories were identified. the therapeutic experiences for repetitive negative thinking included perceived control over behavior against negative thoughts and focus on the present moment; inhibitory factors included environmental factors, particularly friends and family. therapeutic experiences for clinical anger included accepting shame and sorrow, behavioral non-avoidance, and a new understanding of frustrations and unpleasant events. inhibitory factors included the difficulty of transferring learning outside of therapy sessions and others’ expectations for rapid improvement.
