تعارض منافع در نظام سلامت ایران: یک مطالعه مروری

زمینه و هدف: تعارض منافع قرار گرفتن در جایگاهی است که افراد میان منافع شخصی و منافع عمومی قرار گرفته است. این پدیده به‌ویژه در نظام سلامت چالشی بسیار مهم است که می‌بایست ابعاد مختلف و پنهان آن شناسایی شود. این مطالعه به منظور بررسی مقالات منتشر شده از تعارض منافع در حوزه سلامت انجام شده است.روش: مطالعه از طریق مرور ادبیات و متون علمی موجود در حوزه مدیریت، سیاست‌گذاری و اقتصاد سلامت و با بررسی مقالات بیشتر کیفی و منابع موجود در پایگاه‌های اطلاعاتی داخلی و خارجی و با استفاده از کلیدواژه‌های تعارض منافع، نظام سلامت، ایران و conflict of interests health system در محدوده زمانی 10 سال گذشته صورت گرفته است.یافته‌ها: بررسی‌ها در حوزه سلامت نشان می‌دهد اشکال متنوعی از تعارض در نظام سلامت وجود دارد. از قرار گرفتن همزمان در مسند تولیت و سهامداری بخش خصوصی گرفته تا موارد دیگر و از جمله رابطه با شرکت‌های دارویی و تجهیزات پزشکی که همگی به صورت علنی مرتبط با پدیده تعارض منافع است و برای کنترل این موارد و مصادیق سازوکار مدیریتی پیشنهاد شده است.نتیجه‌گیری: در ساختار موجود نظام سلامت به علل مختلف گاهی ترجیحات شخصی به‌ جای معیارها و ارزش‌های عینی، تاثیر کلیدی بر تصمیم‌گیری‌های دولتمردان و در نتیجه تعیین اولویت‌ها دارند. به نظر می‌رسد با مطالعه دقیق‌تر ابعاد موضوع می‌توان موقعیت‌ها و مصادیق مختلف آن را در تمام حوزه‌ها شناسایی و راهکار اجرایی لازم برای مقابله با آن ارائه کرد.

conflict of interest in iran's health system: a review study

background: conflict of interest is being in a position where people are placed between personal interests and public interests. this phenomenon is a very important challenge, especially in the health system, and its different dimensions should be identified. this study was conducted in order to review the published articles on the conflict of interest in the field of health.methods: studying through the review of literature and scientific texts available in the field of management, policy making and health economics and by reviewing mostly qualitative articles and sources available in domestic and foreign databases and using the keywords conflict of interest, health system, iran and conflict of interests health system has taken place in the time frame of the last 10 years.results: surveys in the field of health show that there are various forms of conflict in the health system. from being in the position of private sector ownership and ownership at the same time to other cases, including the relationship with pharmaceutical and medical equipment companies, all of which are publicly related to the phenomenon of conflict of interest, and management mechanism examples have been proposed to control these cases.conclusion: in the existing structure of the health system, for various reasons, sometimes personal preferences instead of objective criteria and values have a key influence on the decisions of statesmen and, as a result, setting priorities. it seems that with a more detailed study of the dimensions of the subject, it is possible to identify its various situations and examples in all fields and provide the necessary implementation solution to deal with it.
