کاوش محرک ها و شرایط خطای انسانی پزشکان در بیمارستان‌های شهرستان رفسنجان

هدف پژوهش حاضر کاوش محرک ها و شرایط خطای انسانی پزشکان در بیمارستان‌های شهرستان رفسنجان است. ابتدا محرک ها و شرایط با توجه به مطالعات کتابخانه ای مورد شناسایی قرار گرفت و در ادامه با استفاده از نظرات خبرگان تا رسیدن به اجماع نظری از روش تکنیک دلفی فازی استفاده شده است. این پژوهش از نظر هدف کاربردی و از نظر جمع‌آوری داده‌ها توصیفی- پیمایشی به شمار می‌آید. جامعه آماری این پژوهش پزشکان بیمارستان‌های شهرستان رفسنجان است. 43 نفر از طریق روش نمونه گیری هدفمند به عنوان حجم نمونه انتخاب شدند. نتایج حاصل از تجزیه و تحلیل داده‌ها نشان داد که در رابطه با محرک ها خطای انسانی پزشکان بیشترین میزان موافقت خبرگان با مولفه­ اطلاعات غلط در مورد بیمار و نحوه نامناسب توزیع دارو و کمترین میزان موافقت با مولفه‌های عدم رعایت حقوق قانونی بیمار و عدم بکار بستن استانداردهای اخلاقی و اصول حرفه­ای است. همچنین در رابطه با شرایط خطای انسانی پزشکان، بیشترین میزان موافقت خبرگان با مولفه ایجاد استرس­های روحی- روانی در پزشکان و افزایش شکایت­ها در بین بیماران و ارباب رجوعان بیمارستان و کمترین میزان موافقت با مولفه‌های تدوین برنامه جامع در خصوص اجرایی کردن فعالیت­­های مرتبط با ارتقاء سلامت، جمع‌آوری و انتشار بهتر و بیشتر اطلاعات و حاکمیت فرهنگ کار تیمی می‌باشد.

exploring the drivers and conditions of human error of doctors in rafsanjan hospitals

the aim of the current research was to explore the drivers and conditions of human error of doctors in rafsanjan hospitals. first, the stimuli and conditions were identified according to library studies, and then the fuzzy delphi technique was used with the help of experts opinions until a theoretical consensus was reached. this research is applied in terms of purpose and descriptive-survey in terms of data collection. the statistical population of this research was the doctors of rafsanjan hospitals. 43 people were selected as the sample size through the purposeful sampling method. the results of the data analysis showed that in relation to the drivers human error of doctors, the highest level of agreement of the experts was with the component of wrong information about the patient and the inappropriate way of drug distribution. and the lowest level of agreement is with the components of not respecting the legal rights of the patient and not applying ethical standards and professional principles. also, in relation to the conditions of human error of doctors, the highest level of agreement of the experts with the component of creating mental-psychological stress in doctors and the increase of complaints among patients and clients of the hospital and the lowest level of agreement with the components of developing a comprehensive plan regarding implementation doing activities related to health promotion, collecting and disseminating better and more information and governing the culture of teamwork. finally, according to the research results, some suggestions have been provided.
