بررسی مهم ترین چالشهای آموزش به بیمار ازدیدگاه پرستاران مجتمع آموزشی درمانی امام علی (ع) کرج سال 1400

آموزش به بیمار، یکی ازاستانداردهای کیفیت مراقبتی وازعوامل تاثیرگذاربرهزینه ها،بهبودی و درمان بیمارمیباشد.آموزش به بیماریکی ازعملکردهای اصلی حرفه های بهداشتی درمانی بوده وفرآیندی بیمارمحورومبتنی برنیازهای ابرازشده توسط پزشک وبیماربرای کمک به بیمارجهت تصمیم گیری مشارکتی وآگاهانه درمورد بیماری است. پرستاران به عنوان بزرگ ترین ومحوری ترین عضوتیم بهداشت ودرمان به شمارمی آیندو یکی ازابعاد مهم مراقبت های پرستاری وازنقش های کلیدی پرستار درارائه خدمات بهداشتی درمانی، آموزش به بیماردرطول بستری است. ازآنجا که پرستاران نقش مهمی درامرآموزش به بیمارداشته ومی توانند با داشتن بهترین عملکرد دراین زمینه، موجبات تغییررفتاروعملکرد بیماررافراهم آورند، بنابراین آگاهی ازنظرات آنان می توانند درشناسایی موانع آموزش وکمک به برنامه ریزی وارتقاء برنامه آموزش به بیمارموثر می باشد. آموزش بیماردربیمارستان کاردشواری است وموانع زیادی درمسیرآن وجوددارد.بیماران ممکن است به دلیل اضطراب زیاد،وضع جسمی نامناسب ویا عدم درک اهمیت ونقش آموزش دربیماری خود تمایلی به یادگیری نداشته باشندهمچنین بسیاری ازپرستاران آگاهی ومهارت کافی درمورد روشهاواصول آموزش به بیماررا ندارند. این مطالعه با هدف بررسی مهم ترین چالشهای آموزش به بیمارازدیدگاه پرستاران بیمارستان امام علی (ع) انجام شد. روش کار: مطالعه حاضر یک مطالعه کیفی است که با همکاری 120 پرستار شاغل دربخشهای بزرگسال بیمارستان امام علی درسال 1400 انجام شد. روش نمونه گیری تصادفی و روش جمع آوری اطلاعات با استفاده ازچند سوال با پاسخ باز در حیطه های مدیریتی، سازمانی، مربوط به بیمار و پرسنل انجام شد . (نمونه سوالات : نظرتان درمورد فرآیند آموزش به بیمار، چالشهای آموزش به بیمار از نظر شما، پیشنهادات بهبود روند آموزش به بیمار، مهم ترین موانع بازدارنده آموزش به بیمار و...). سپس اطلاعات جمع آوری شده تجزیه و تحلیل و جمع بندی گردید. یافته ها : نتایج حاصل ازتجزیه وتحلیل داده ها نشان داد که چالش های آموزش به بیمارمتاثرازعوامل مختلفی چون : -موانع مدیریتی (45%) : کم لطفی در زمینه اهمیت آموزش – عدم همکاری برخی پزشکان -موانع سازمانی (55%) : کمبود نیروی پرستاری- کمبود وقت- حجم کار بالا - تجهیزات ناکافی در امرآموزش ازقبیل فلش، فیلم و... -موانع مربوط به بیمار و همراهان (50%) : پایین بودن سطح دانش و سواد بیماران - کهولت سن اکثربیماران - اضطراب و نگرانی از بستری و روند درمان – اختلاف فرهنگی – همراه نا آگاه - بی حوصلگی بیمارو... -موانع پرسنلی (35%) : بی اعتمادی رابطه – انگیزه کمرنگ پرسنل به دلیل تشویق نامناسب ازطرف مدیران مجموعه – اختلاف زبان بیماران و پرسنل – فهم دشواربه دلیل ناتوانی جسمی بیماران ( کم شنوایی، نابینایی و...) که از این میان نقش سازمان (کمبود نیرو و تجهیزات) و موانع مربوط به بیمار محسوس ترمی باشد.لذا جهت بهبود وضعیت آموزش به بیمار، پیشنهاد می شود مسئولین بیمارستانها علاوه برارزشیابی پرسنل نسبت به این امرمهم،جهت گنجاندن شرح وظایف خاص دراین رابطه وهمچنین تعیین امکانات بیشتربرای آموزش به بیمارتوجه بیشتری مبذول دارند. با برقراری دوره های آموزشی،دانش و مهارت پرسنل پرستاری رابرای شناخت نیازهای یادگیری بیماروهمچنین برقراری ارتباط مناسب بابیمارافزایش داده شود. نتیجه گیری : با توجه به نقش بسیارمهم علاقه و مشارکت بیمارو همچنین ارتباط خوب پرستاربا بیمار،استفاده ازروشهای دیداری وشنیداری می تواند سبب افزایش میزان مشارکت وتسهیل درشناخت نیازهای یادگیری بیمار گردد. در حال حاضردربسیاری از بیمارستانها شاهد ارتقا روز به روز امرآموزش به بیمار می باشیم اما هنوز دربرخی مراکز برنامه آموزش به بیمار بصورت منسجم ویکپارچه و با برنامه مشخص اجرا نمی شود. لذا بایستی مقوله آموزش به بیمار که می تواند تحول اساسی دربهبود کیفیت واثربخشی درنظام سلامت کشور ایجاد نماید، با درنظرگرفتن وتامین نیروی انسانی، تامین بودجه لازم و همراهی مدیران مورد عنایت ویژه قرارگیرد.

examining the most important challenges of patient education from the point of view of the nurses of imam ali (as) karaj educational and therapeutic complex in 1400

patient education is one of the standards of care quality and factors affecting the costs, recovery and treatment of the patient. patient education is one of the main functions of health care professions, and it is a patient-centered process based on the needs expressed by the doctor and the patient to help the patient make informed and collaborative decisions about the disease. nurses, as the largest and most central members of the health and treatment team, are one of the most important aspects of nursing care and the nurse s key roles in providing health care services, teaching the patient during hospitalization. since nurses have an important role in patient education and they can bring about changes in patient behavior and performance by having the best performance in this field, therefore knowing their opinions can be effective in identifying obstacles to education and helping to plan and improve the patient education program. patient education in the hospital is a difficult task and there are many obstacles in its path. patients may not be willing to learn due to high anxiety, inappropriate physical condition, or lack of understanding of the importance and role of education in their illness. also, many nurses do not have sufficient knowledge and skills about the methods and principles of patient education. this study was conducted with the aim of examining the most important challenges of patient education from the perspective of nurses at imam ali hospital (as). methodology: the present study is a qualitative study that was conducted with the cooperation of 120 nurses working in the adult wards of imam ali hospital in 1400. random sampling method and data collection method was done using several questions with open answers in managerial, organizational, patient and personnel areas. (sample questions: your opinion about the patient education process, the challenges of patient education in your opinion, suggestions for improving the patient education process, the most important obstacles preventing patient education, etc.). then the collected information was analyzed and summarized. findings: the results of the data analysis and analysis showed that the challenges of patient education are affected by various factors such as: - management obstacles (45%): lack of favor in the field of the importance of education - non-cooperation of some doctors - organizational obstacles (55%): lack of nursing staff - lack of time - high workload - inadequate equipment for training, such as flash, video, etc. - obstacles related to the patient and companions (50%): low level of knowledge and literacy of patients - old age of most patients - anxiety and worry about hospitalization and treatment process - cultural difference - uninformed companion - patient s impatience, etc. - personnel obstacles (35%): lack of trust in the relationship - poor motivation of the staff due to inappropriate encouragement from the complex managers - language difference between patients and staff - difficult understanding due to physical disability patients (hearing loss, blindness, etc.), among which the role of the organization (lack of manpower and equipment) and the obstacles related to the patient are more noticeable. therefore, in order to improve the education situation for the patient, it is suggested that the hospital officials, in addition to evaluating the personnel in relation to this important matter, in order to include the description of specific duties in this regard and also to determine more facilities for patient education, they pay more attention. by establishing training courses, the knowledge and skills of the nursing staff should be increased to understand the patient s learning needs and also establish proper communication with the patient. conclusion: considering the very important role of the patient s interest and participation, as well as the good relationship between the nurse and the patient, the use of visual and auditory methods can increase the participation rate and facilitate understanding of the patient s learning needs. currently, in many hospitals, we see the improvement of patient education day by day, but in some centers, the patient education program is not implemented in a coherent and integrated manner and with a specific program. therefore, the category of patient education, which can create a fundamental change in improving the quality and effectiveness of the country s health system, should be given special attention by considering and providing human resources, providing the necessary budget and accompanying managers.
