massive peripheral arterial thrombosis in a case of sars-cov-2 in iran

Little is known about the pathophysiology and thrombosis of the covid-19 disease, even though abnormal coagulation and cardiovascular involvement are increasing medical concerns. a 61-year-old male was admitted with fever and productive cough (in fasa province, iran). he was positive for the sars-cov-2 test using real-time rt-pcr. after 12 days of hospitalization, his left leg became pale and cold, the pulse of the femoral artery was diminished and he had a sudden onset of severe pain. on color doppler sonography, there was evidence of extensive arterial thrombosis in the left side of common femoral, superficial femoral, deep femoral, popliteal, anterior tibialis, and dorsal pelvic arteries. then, he received an emergency thrombectomy. the histopathological finding of the clot is shown in fig. 1. lupus anticoagulant (la) antibody positive was also diagnosed in this patient. increasing lupus anticoagulant (la) antibody in covid-19 positive patients may be considered as a disposing factor in the thrombotic events. it is suggested that the la antibody should be checked in covid-19 patients to administer prophylactic anticoagulant drugs to prevent thrombosis.
