بررسی تحلیلی خدمات مامایی و سلامت باروری در ایران

مقدمه و هدف: برای ارتقای کیفیت خدمات سلامت، بررسی وضعیت موجود ارائه خدمات، امری ضروری است. در این مطالعه وضعیت ارائه خدمات کنونی بهداشت باروری و مامایی در بعد خدمات- نیروی انسانی و مدیریت و برنامه‌ریزی تبیین شد. روش کار: پس از کسب مجوز اخلاق برای اجرای پژوهش،‌ مطالعه به صورت کیفی با رویکرد تحلیل محتوای جهت‌دار انجام شد، مصاحبه‌های نیمه ساختاریافته با 47 نفر از ارائه‌دهندگان و برنامه‌ریزان و مدیران خدمات بهداشت باروری و مامایی، اخذ شد. نتایج بدون نرم‌افزار به روش گرانهایم و لاندمن تجزیه و تحلیل شد. نتایج: وضعیت خدمات بهداشت باروری در قالب 3 درون مایه اصلی، خدمت، ارائه‌دهنده خدمت و مدیریت خدمت متشکل از 12 طبقه ، 36 زیر طبقه فرعی اصلی، 101 زیر طبقه فرعی و 271 کد نهایی به تفصیل مورد بررسی قرار گرفت. طبقات به ترتیب در درون‌مایه خدمت شامل پوشش‌دهی،‌ تناسب ساختاری، فرآیند و پیامد، در درون‌مایه ارائه‌دهنده خدمت شامل مقرون به صرفه بودن نیرو، اختیار و محدوده عملکرد، کارتیمی، ‌برنامه‌ریزی مطلوب، چالش‌های نیروی‌کار و بهسازی نیرو و در درون‌مایه مدیریت خدمت شامل فراهمی زیرساخت‌های مناسب و وظایف مدیر بود. نتیجه گیری: عمده‌ترین چالش‌های کنونی شامل عدم وجود الگوی مراقبتی پیوسته در بهداشت باروری، عدم وجود نظام سطح‌بندی مناسب، عدم توجه به مفهوم هزینه اثربخشی، محدوده نامشخص وظایف‌ حرفه‌ای، روحیه ضعیف کار تیمی، نبود شاخص‌های مناسب برای محاسبه نیروی انسانی ماما و ضعف در نظارت بر ارائه خدمات بود. برطرف کردن نقص‌های کنونی در ارائه خدمات و رفع کمبودهای موجود در نیروی انسانی مامایی به بهبود کیفیت ارائه خدمات بهداشت باروری می‌انجامد.

introduction and purpose: to improve the quality of health services, it is necessary to examine the current situation of service provision. in this study, the current state of providing reproductive health and midwifery services was explained in terms of services, human resources, management and planning. methodology: after obtaining ethics permission to conduct the research, the study was conducted qualitatively with a directed content analysis approach, semi-structured interviews were conducted with 47 providers, planners, and managers of reproductive health services, and the results were obtained without software using the granheim method. and lundman analysis. results: the status of reproductive health services in the form of 3 main themes, service, service provider and service management consisting of 12 classes, 36 main sub-themes, 101 sub-themes and 271 final codes were examined in detail. the categories are respectively in the content of the service, including coverage, structural fit, process and outcome, in the content of the service provider, including the cost-effectiveness of the force, authority and scope of performance, efficiency, optimal planning, challenges of the workforce and improvement of the force, and in the content of the service management, including availability adequate infrastructure and manager s duties. conclusion: the main current challenges include the absence of a continuous care model in reproductive health, the absence of a suitable leveling system, the lack of attention to the concept of cost-effectiveness, the unclear scope of professional duties, the weak spirit of team work, the lack of suitable indicators for calculating midwifery manpower and the weakness in it was monitoring the provision of services. fixing the current deficiencies in providing services and solving the shortages in midwifery manpower leads to improving the quality of providing reproductive health services.
