   identification and analysis of human error for an explosives production facility: application and comparison of teseo, heart and ahp integrated heart methods

چکیده    This study identified possible risk factors that may cause accidents by the hazard and operability analysis (hazop) in an explosion production facility to enhance safety and operational reliability in the explosive industry. then, human-induced errors were determined among these risk factors. these human errors were quantified using the tecnica empirica stima errori operatori (teseo), the human error evaluation and reduction technique (heart), and the analytical hierarchy process (ahp) integrated heart methods. the approaches were interpreted by comparing them with each other. as a result, the probability of human error was more significant for the heart method in the vast majority. this is because, compared to the teseo approach, which allows for rapid analysis, the heart method considers various environmental and task-related working conditions that may cause human error. it was concluded that integrating the ahp technique into the heart method contributed to the purification of the subjectivity of the heart method, which includes expert judgments.
