ارتقاء یادگیری جراحی آپیکال در دستیاران اندودانتیک دانشکده دندانپزشکی مشهد با استفاده از مدل فک گوسفند

مقدمه: طبق بررسی های متعدد مشخص شد که دستیاران درمان ریشه در زمینه انجام جراحی های آپیکال مشکلاتی داشتند و بدون هیچ آموزش عملی و صرفا بر اساس اندوخته های نظری ، در سال سوم دستیاری جراحی آپیکال را آغاز میکردند که موجب کاهش کیفیت جراحی ها، شکست های بالای درمان ها، استرس بالا و کاهش انگیزه و اعتماد به نفس دستیاران ، طولانی شدن عمل های جراحی، اذیت شدن بیمار و گاها ایجاد خطا های جبران ناپذیر بر روی بیمار می شد. بنابراین تصمیم گرفته شد که گام هایی پیوسته جهت رفع این مشکلات از مرحله پره کلینیک آغاز شود و تا مراحل کلینیک و کار بر روی بیمار  ادامه یابد. هدف از انجام مطالعه حاضر، بررسی ارتقاء مهارت ، توانمندی و انگیزه دستیاران رشته درمان ریشه در انجام جراحیهای پری آپیکال بعد از اعمال تغییرات در پره کلینیک درمان ریشه بود.روش کار: سال 1395 تغییراتی در مدل و نحوه آموزش جراحی آپیکال در بخش درمان ریشه دانشکده دندانپزشکی مشهد انجام شد که بر اساس نظرسنجی هایی از اساتید و دستیاران ، همچنین بررسی کیفیت درمان های انجام شده، کارآیی این سیستم جدید ارزیابی شد. بنابراین مطالعه ای توصیفی - مقطعی طراحی گردید. جهت بررسی میزان رضایتمندی دستیاران ،پرسشنامه ها بین دستیاران درمان ریشه دانشکده دندانپزشکی مشهد از ورودی1395 تا 1399 توزیع شد و به منظور ارزیابی اساتید از نحوه عملکرد دستیاران، پرسشنامه ها به 13 نفر هیات علمی بخش درمان ریشه داده شد. قبل از توزیع پرسشنامه ها روایی صوری و محتوایی آنها بررسی شد و پایایی آنها با روش آزمون - باز آزمون تعیین گردید. نمونه گیری به صورت همه پرسی انجام شد.نتایج: نتایج نظرسنجی از دستیاران، نشان از افزایش 78/3% کیفیت درمان ها، 65% در سرعت عمل و افزایش اعتماد به نفس، و 78/3 % در انگیزه انجام عمل های جراحی پری آپیکال به طور مستقل بود. بر اساس نظر اساتید، تسلط و مهارت ، موفقیت درمان ها، نمرات در سمینارهای ارائه caseجراحی 100 افزایش داشت و سرعت عمل ، دستیاران و همچنین رضایتمندی بیماران به میزان 70% افزایش یافت.نتیجه گیری: ارایه این مدل آموزشی جراحی پری آپیکال، نقش مهمی در ارتقا آموزش دستیاران درمان ریشه دارد و میتواند در کوریکولوم آموزشی وارد شود.

improvement of apical surgery training in endodontic assistants on mashhad dental school by jaw lamb model

introduction: several investigation have shown that endodontic assistants had problems in performing apical surgeries and without any practical training and only based on theoretical reserves, they started apical surgery in the third year, which caused reduction of the quality of surgeries, high treatment failures, high stress and reduced motivation and self-confidence of residents, prolongation of surgeries, patient harassment and sometimes irreparable mistakes were made on the patient. therefore, it was decided that continuous steps to solve these problems should start from the pre-clinic and continue until the clinical stage. the aim of the present study was to investigate the improvement of skills, competence and motivation of endodontic assistants in performing periapical surgeries after changes in the pre-clinic of endodontic treatment.materials methods: since 2016, changes have been made in the model and method of teaching in apical surgery in the endodontic department of mashhad dental school. based on surveys of endodonticprofessors and assistants, as well as reviewing the quality of treatments, the effectiveness of this new system was evaluated. therefore, a descriptive cross-sectional study was designed. to assess the satisfaction of residents, questionnaires were distributed among the endodontic residents of mashhad dental school from the entrance of 2016 to 2020, and another questionnaires were given to 13 faculty members of the endodontics department. before distributing the questionnaires, their validity and reliability were checked.results: the results of a survey from residents showed 78.3%  increase the quality of treatments, 65% speed of operation and confidence and motivation to perform periapical surgeries independently.according to the professors, the speed of action, mastery and skills of the assistants as well as the patients’ satisfaction have increased.conclusion: providing this educational model of periapical surgery has an important role in promoting the training of endodontic assistants and can be included in the educational curriculum.
