بررسی فاکتورهای مرتبط با آمادگی ترخیص بیماران هوشیار در حال ترخیص در بخش های عمومی: یک مطالعه مقطعی

مقدمه و هدف: انتقال مراقبت از بیمارستان به منزل، نقطه‌ای بحرانی در تداوم زنجیره مراقبت‌های بالینی است که ممکن است افراد را در معرض پیامدهای نامطلوب قرار دهد. آمادگی ترخیص بیمار از بیمارستان می‌تواند منجر به کاهش نگرانی‌های مرتبط با مراقبت بیمار از خود پس از ترخیص شود. بنابراین، شناسایی فاکتورهای مرتبط با آمادگی ترخیص از بیمارستان حائز اهمیت است. مطالعه حاضر با هدف تعیین فاکتورهای مرتبط با آمادگی ترخیص بیماران هوشیار در بخش‌های عمومی طراحی شد.روش کار: این مطالعه مقطعی روی 351 بیمار هوشیار در حال ترخیص در بیمارستان های آموزشی دانشگاه علوم پزشکی کردستان انجام شد. روش نمونه‌گیری سهمیه‌ای برای تعیین تعداد بیماران منتخب از هر بیمارستان و روش نمونه گیری در دسترس برای انتخاب بیماران هوشیار درحال ترخیص استفاده شد. ابزار جمع‌آوری داده‌ها شامل فرم اطلاعات دموگرافیک و پرسشنامه استاندارد آمادگی ترخیص از دیدگاه بیمار بود. تحلیل داده‌ها با نرم‌افزار spss-21  و با سطح معناداری 0/05 انجام شد.یافته ها: میانگین نمره کل آمادگی ترخیص از بیمارستان بیماران مشارکت کننده در مطالعه 10/31±47/63 بود. آمادگی ترخیص با متغیرهای سن (0/001>p)، جنس (0/05>p)، تحصیلات (0/001>p)، شغل (0/001>p)، تاهل (0/05>p)، منبع حمایتی در منزل (0/001>p)، مدت زمان بستری (0/001>p)، درآمد ماهیانه (0/05>p) و سطح مراقبتی (0/001>p) دارای ارتباط معنی دار بود. نتیجه‌گیری: مطابق با نتایج مطالعه میانگین آمادگی ترخیص بیماران در سطح پایین بود. از سویی دیگر توجه به فاکتورهای موثر بر آمادگی ترخیص بیماران و برنامه ‌ریزی و نظارت مناسب بر فرآیند ترخیص برای دستیابی به سطح رضایت ‌بخش آمادگی ترخیص بیماران ضروری است.

investigating factors associated with discharge readiness of conscious patients being discharged from general wards: a cross-sectional study

introduction: the transition of care from hospital to home is a critical point in the continuum of clinical care that may expose individuals to adverse outcomes. patient preparedness for discharge from hospital can reduce concerns related to patient self-care after discharge. therefore, identifying factors associated with hospital discharge readiness is important. the present study was designed to determine factors associated with discharge readiness of conscious patients in general wards.materials and methods: this cross-sectional study was conducted on 351 conscious patients being discharged from teaching hospitals of kurdistan university of medical sciences. a quota sampling method was used to determine the number of patients selected from each hospital and a convenience sampling method was used to select conscious patients being discharged. the data collection tools included a demographic information form and a standard questionnaire on discharge readiness from the patient’s perspective. data analysis was performed with spss-21 software with a significance level of 0.05. results: the mean total discharge readiness score of the patients participating in the study was 47.63±10.31. discharge readiness was significantly associated with the variables of age (p<0.001), gender (p<0.05), education (p<0.001), occupation (p<0.001), marriage (p<0.05), source of support at home (p<0.001), length of hospitalization (p<0.001), monthly income (p<0.05), and level of care (p<0.001).conclusion: according to the results of this study, the average level of patient discharge readiness was low. on the other hand, paying attention to the factors affecting patient discharge readiness and proper planning and monitoring of the discharge process are essential to achieving a satisfactory level of patient discharge readiness.
