Asthma is increasingly recognized not only as a chronic inflammatory airway disease, but also as a potential contributor to systemic vascular changes and cardiovascular risks. emerging evidence suggests a link between asthma and arterial injury; yet, pediatric data remain scarce and inconsistent. the objective of this study is to assess the levels of serum anti-elastin antibodies and carotid intima-media thickness (cimt) in asthmatic children compared to healthy controls and, secondarily, to examine their relationship with levels of asthma control. a prospective case-control study was conducted at children's hospitals, al-azhar university in cairo, al-azhar faculty of medicine in assiut, aswan university egypt and the faculty of medicine ain shams university between december 2023 and april 2025. it included 35 asthmatic children aged 4–16 years (group a) and 35 healthy, age- and sex- matched controls (group b). the collected data included clinical history, anthropometry, vital signs, laboratory investigations (including serum anti-elastin antibodies, and total ige), and carotid ultrasonography for cimt. asthma control was assessed according to gina 2023 guidelines. asthmatic children showed significantly higher levels of serum anti-elastin antibodies. total serum ige levels also demonstrated a strong positive correlation with anti-elastin antibody levels.this study highlights serum anti-elastin antibodies as a potential biomarker for the presence and severity of asthma in children. although no significant differences in carotid intima-media thickness were observed, the elevated antibody levels in uncontrolled asthma cases indicate an ongoing inflammatory and possibly vascular process.
