The lewis (le) blood group system, unlike other systems, depends on soluble glycan antigens synthesized by fucosyltransferase enzymes fut2 and fut3. these antigens are implicated in host–microbe interactions, including helicobacter pylori adhesion. despite extensive global research, data on the relationship between lewis and abo blood groups, and h. pylori infection among patients remain limited. the objective of this study is to evaluate the correlation between fut2 (secretor) and fut3 (lewis) enzyme activity, h. pylori infection, smoking, and fast-food intake among patients presenting with dyspepsia. a cross-sectional study was conducted at al-azhar university hospitals between february and august 2025, including 60 dyspeptic patients. each participant provided blood, stool, and gastric biopsy samples for h. pylori detection and blood group phenotyping (abo, rh, and lewis). data were analyzed using spss version 26. quantitative variables were presented as mean ± sd, and qualitative data as frequency and percentage. associations were evaluated using chi-square tests, with p ≤ 0.05 considered statistically significant. the most frequent lewis phenotype was le(a−b+) (42%). statistical analysis revealed no significant overall association between h. pylori infection and lewis phenotype distribution (p = 0.23). however, the interpretation of this finding is limited by the observation of a biologically implausible le(a+b+) phenotype in 15% of the cohort, suggesting a potential methodological anomaly in phenotyping. no significant associations were found with abo blood group (p = 0.32), smoking (p = 0.60), or fast-food intake (p = 0.77). no statistically significant relationships were observed between h. pylori infection and abo blood group antigens, smoking, or fast-food intake among dyspeptic patients. the interpretation of the relationship with lewis antigens is confounded by the unexpected detection of the le(a+b+) phenotype, indicating a potential limitation in the phenotyping method. the findings suggest that environmental and social determinants may play a more prominent role in h. pylori transmission than the genetic or lifestyle factors measured here.
