   association between lv systolic function by two-dimensional speckle tracking echo and triglycerides–glucose index in obese patients

چکیده    Obesity is linked to insulin resistance and adverse cardiovascular events. the connection between the tyg index and left ventricular (lv) systolic function in obese individuals remains ambiguous. therefore, this study aimed to examine the relationship between the tyg index and lv systolic function, as evaluated by 2d-ste, in obese women. in this prospective cross-sectional study, 90 women aged 35–45 years were enrolled and divided into two groups: 55 obese (bmi ≥30 kg/m²) and 35 non-obese controls (bmi 18.5–24.9 kg/m²). all participants underwent clinical evaluations, anthropometric measurements, fasting lipid profiles, and fasting glucose and insulin tests. the tyg index was calculated as ln [fasting triglycerides (mg/dl) × fasting glucose (mg/dl)/2]. cardiac assessments included conventional transthoracic echocardiography, tissue doppler imaging, and 2d-ste to evaluate lv global longitudinal strain.obese patients exhibited significantly higher tyg index values compared to controls  with larger lv and la dimensions although lv ejection fraction was unchanged. lv gls was marginally lower in the obese group , but the difference was not statistically significant.a significant negative correlation was observed between the tyg index and lv gls . conventional echocardiography showed preserved lv function in obese patients, strain imaging revealed subtle impairments. the tyg index was elevated in obese participants and negatively correlated with lv gls. this association suggests it could be a useful marker for identifying subclinical lv systolic impairment in obesity and warrants further investigation in longitudinal studies to determine its value for early risk stratification.
