state of coagulation in pre-eclampsia: comparing sonoclot® signature analysis with conventional hemostasis assays

Preeclampsia (pe) is a significant health concern for pregnant women and their babies, contributing to high rates of morbidity and mortality. the sonoclot analyzer is a point-of-care device that detects viscoelastic changes in whole blood after activation. this study aims to evaluate sonoclot parameters in preeclamptic patients and correlate them with conventional hemostasis assays. this case-control study enrolled a total of 74 women, categorized into three groups: 34 women with preeclampsia, 30 normotensive pregnant women, and 10 healthy non-pregnant women. all participants underwent clinical examination and laboratory investigations, including conventional coagulation tests (pt, aptt, protein c, protein s, and antithrombin iii) and sonoclot signature analysis (act, clot rate, and platelet function). pre- eclamptic women exhibited significantly elevated blood pressure and white blood cell counts compared to the other groups. the coagulation profile indicated a hypercoagulable state in pre- eclampsia, with a significantly accelerated sonoclot clot rate and reduced antithrombin iii levels. the sonoclot platelet function parameter showed no significant differences among the groups. the sonoclot parameter act (activated clotting time) showed a significant positive correlation with aptt. unlike conventional tests, act also correlated positively with natural anticoagulants, particularly protein s, in the pre- eclamptic group. pre-eclampsia is characterized by a hypercoagulable state and a high prevalence of acquired protein s deficiency. sonoclot analysis correlates with conventional coagulation assays and reflects changes in natural anticoagulants, suggesting it is a useful tool for evaluating the coagulation dynamics in pre-eclamptic women.
