protective effects of hypercapnic preconditioning on endothelial dysfunction, inflammation, and oxidative stress in myocardial ischemia-reperfusion injury

This experiment evaluated the cardioprotective potential of hypercapnic preconditioning (exposure to 5%, 10%, and 15% co2) against myocardial ischemia-reperfusion injury (miri) in male wistar rats (n = 6/group), focusing on biomarkers of endothelial dysfunction, inflammation, and oxidative stress. rats were exposed to normoxic gas mixtures with graded co2 concentrations (0% [control], 5%, 10%, or 15%) for four hours, followed by 30-minute left anterior descending coronary artery occlusion and 120-minute reperfusion. serum analysis post-intervention demonstrated dose-dependent effects: endothelin-1 (et-1), a vasoconstrictive peptide, was significantly reduced in the 10% co2 group compared to controls (p < 0.001). tumor necrosis factor-alpha (tnf-α), a pro-inflammatory cytokine, exhibited a marked decrease in the 15% co2 group (p < 0.05). superoxide dismutase (sod) activity progressively declined across all co2-treated groups (p < 0.05 vs. control), with the lowest levels at 15% co2, potentially reflecting diminished reactive oxygen species (ros) generation or altered antioxidant dynamics. these results suggest that hypercapnia attenuates miri via distinct mechanisms-suppressing et-1-mediated vasoconstriction, reducing tnf-α-driven inflammation, and modulating oxidative stress responses. controlled co2 elevation before ischemia may thus serve as a therapeutic adjunct to reperfusion strategies. further research is warranted to optimize co2 dosing and timing protocols for clinical translation, particularly in cardiac surgery where ischemic preconditioning could enhance outcomes by targeting multiple injury pathways simultaneously.
