clinicopathological characteristics of deep infiltrating endometriosis undergoing surgical therapy at rsud dr. soetomo (2022-2024)

Deep infiltrating endometriosis (die) is a chronic and aggressive form of endometriosis characterized by lesions invading more than 5 mm beneath the peritoneal surface. this study aims to evaluate the role of clinical findings, physical examination, and radiological assessments (ultrasonography and mri) in diagnosing die, and to identify potential risk factors associated with disease progression. a retrospective observational study with a cross-sectional design was conducted on patients diagnosed with die and who underwent surgical treatment at rsud dr. soetomo, surabaya, between 2022 and 2024. a total of 38 patients with complete medical records and histopathological confirmation were included. no statistically significant relationships were found between die and age, parity, body mass index (bmi), history of infertility, dysmenorrhea, dysuria, dyschezia, dyspareunia, pain severity (vas score), or menstrual pattern (p > 0.05). however, multivariate logistic regression analysis demonstrated that the absence of prior hormonal therapy was significantly associated with an increased odds of histologically confirmed deep infiltrating endometriosis (die), with an odds ratio (or) of 5.476 (95% ci: 1.038–28.879; p < 0.05). radiological evaluation showed variable accuracy, with no significant correlation found between preoperative imaging and intraoperative or histopathological findings. histopathology remains the gold standard. this study highlights the protective role of hormonal therapy in reducing the risk of die development, suggesting its potential benefits in early management strategies. further prospective studies are needed to validate these findings and optimize diagnostic and therapeutic approaches.
